粘附分子与老年人非小细胞肺癌血行转移关系的探讨 被引量:9

The relationship between the peripheral blood expression of adhesion molecules and hematogenous metastasis in elderly patients with non-small cell lung cancer

摘要 目的 研究老年人非小细胞肺癌血行转移时粘附分子的作用。 方法 应用流式细胞荧光标记法 ,检测不同分期的老年非小细胞肺癌患者 (老年肺癌组 )外周血中CD4 4、CD5 4和CD6 2P表达水平 ,与健康老年人、肺部良性病变患者及非老年肺癌患者进行对照 ,并在应用复方丹参后观察粘附分子表达水平的变化。 结果 老年肺癌组外周血粘附分子CD4 4、CD5 4和CD6 2P的表达水平〔分别为 (5 4 9 2 3± 13 4 4 )、(2 6 7 5 6± 12 0 5 )、(6 0 3 2 2± 10 1 2 7)〕明显高于健康对照组及良性病变组 (P <0 0 1) ,与非老年肺癌组比较 ,差异无显著性。CD4 4、CD5 4和CD6 2P的表达水平与老年肺癌患者病理类型无相关 ,但与临床分期相关 ,Ⅲ、Ⅳ期的老年肺癌患者 3种粘附分子表达水平〔分别为 (5 92 34±16 2 6 )、(2 79 15± 16 5 5 )、(74 6 5 8± 12 1 6 2 )〕与Ⅰ、Ⅱ期老年肺癌患者〔分别为 (486 18± 18 73)、(2 4 7 6 5± 10 87)、(392 73± 99 35 )〕差异有显著性 (P <0 0 5 )。复方丹参具有明显抑制粘附分子CD4 4、CD4 5和CD6 2P表达的作用。 结论 粘附分子在介导老年人非小细胞肺癌细胞血行转移中可能起着重要作用 ,其表达水平的变化可为临床判断患者疗效、预后提供依据。 Objective To investigate the mechanism of the adhesion molecules in peripheral blood in hematogenous metastasis of elderly patients with non small cell lung cancer. Methods Peripheral blood expression of CD44, CD54 and CD62P in different 53 elderly patients with non small cell lung cancer were detected by immune flow cytometry, 31 normal elderly and 35 elderly patients with benign lesions and 42 non elderly lung cancer served as controls. The effect of Salvia miltiorrhiza complex(SMC) treatment on the expression of the adhesion molecules in hematogenous metastasis was observed. The student's t test was used for statistical analysis in the study. Results The expression of CD44 and CD54 in peripheral lymphocyte and CD62P in platelet of the elderly non small cell lung cancer were 549 23±13 44, 267 56±12 05 and 603 22±101 27, respectively, significantly higher than those in the normal elderly controls or the benign lesion controls( P <0 01). There were no significant difference between CD44, CD54 and CD62P in elderly lung cancer and non elderly lung cancer controls( P >0 05). The expression of CD44, CD54 and CD62P was not related to the pathological typing( P >0 05), but closely associated with the clinical staging( P <0 05). The expression in stage Ⅰ and stage Ⅱ of the elderly lung cancer were 486 18±18 73, 247 65±10 87, 392 73±99 35, respectively, significantly lower than those in stage Ⅲ and stage Ⅳ 〔(592 34±16 26), (279 15±16 55), (746 58±121 62)〕. After SMC treatment, it showed no distinct difference between CD44 or CD54 expression in elderly lung cancer and the normal controls( P >0 05). But CD62P expression in elderly lung cancer after SMC treatment was significantly higher than that in normal controls ( P <0 01). Conclusions The adhesion molecules in peripheral blood of the non small cell lung cancer patients played an important role in hematogenous metastasis. The change of its expression could provide a clinical proof to judge the curative effect and prognosis. SMC showed antimetastatic effect on aged lung cancer and its mechanism might be associated with its inhibition in the expression of CD44, CD54 and CD62P in peripheral blood.
