摘要
目的:探讨糖尿病周围神经病变(DPN)不同中医证型与肌电图神经传导速度的关系。方法:符合纳入标准的103例DPN患者,随机分为气阴两虚(A组)、痰瘀阻络(B组)、阳虚寒凝(C组)、肝肾亏虚(D组)四组。比较四组患者正中神经、尺神经、胫神经、腓总神经的运动末端MCV、SCV及其AMP差异。结果:1)正中神经及腓总神经的MCV在痰瘀阻络型、阳虚寒凝型、肝肾亏虚型与气阴两虚型比较差异具有统计学意义(P<0.01)。2)正中神经、胫神经及腓总神经的SCV在痰瘀阻络型、阳虚寒凝型、肝肾亏虚型与气阴两虚型比较,差异具有统计学意义(P<0.01)。肝肾亏虚型与气阴两虚型比较,正中神经、尺神经、胫神经、腓总神经的SCV差异均有统计学意义(P<0.05)。3)正中神经、胫神经及腓总神经的AMP提示,肝肾亏虚型与气阴两虚型比较,差异具有统计学意义(P<0.05)。结论:MCV、SCV以及AMP在DPN不同证型中的变化具有差异,而这种差异可以作为中医辨证分型的客观依据。
Objective：To investigate the relationship between TCM and EMG nerve conduction velocity of dia-betic peripheral neuropathy （ DPN） .Methods：103 patients with DPN syndrome type were divided into Qi and Yin deficiency group（ A group） ,phlegm and blood stasis group（ B group） ,Yang deficiency and cold group（ C group） , liver and kidney deficiency group（ D group） .Compare the median nerve, ulnar nerve, tibial nerve, motor end peroneal nerve MCV, SCV and AMP.Results：1） As to MCV in median nerve,collaterals tibial nerve and peroneal nerve, there was a significant difference in A group（ P〈0.01） ,compared with B group,C group and D group.2） As to the SCV in median nerve and peroneal nerve,there was a significant difference in A group（P〈0.01）,compared with B group,C group and D group.As to the SCV in median nerve, ulnar nerve, tibial nerve and peroneal nerve, there was a statistical difference （ P 〈0.05 ） when we compared D group with A group.3）As to AMP in median nerve,collaterals tibial nerve and peroneal nerve,there was a sta-tistical difference （P〈0.01） when we compared D group with A group.Conclusion： MCV, SCV and AMP changes in different syndromes in DPN have differences, and the differences can be used as an objective basis for TCM typing.
