摘要
目的探讨三种治疗方案对腹腔镜手术治疗子宫内膜异位症后患者妊娠的影响。方法选取123例子宫内膜异位症伴不孕患者为研究对象,均给予腹腔镜手术,采用简单抽样法随机分成A、B、C三组,每组41例。A组给予注射用醋酸亮丙瑞林微球皮下注射方案,B组采用口服孕三烯酮胶囊方案,C组给予期待治疗方案。行为期2年的随访,观察比对三组患者治疗后复发及妊娠情况。结果 1随访2年内,A组总妊娠率为78.05%,与B组的73.17%对比无明显差异(P>0.05);A、B两组与C组的43.90%对比差异明显(P<0.05);2随访2年内,A、B两组各复发3例,复发率为7.32%,对比无统计学意义(P>0.05);与C组的26.83%对比差异明显(P<0.05);3Ⅰ期的53例患者随访2年内总妊娠率为64.15%,与Ⅱ期的61.91%对比无明显差异(P>0.05);Ⅳ期的7例患者中2例于2年内妊娠,妊娠率为28.57%,与Ⅲ期的47.62%对比无差异明显(P>0.05),与Ⅰ、Ⅱ期患者对比差异明显(P<0.05)。结论对子宫内膜异位症伴不孕患者给予注射用醋酸亮丙瑞林微球皮下注射方案,疗效确切,可有效提升患者妊娠率,值得临床推广。
Objective To explore the influence of pregnancy in the treatment of laparoscopic surgery on endometriosis patients after three kinds of treatment.Methods Selected 123 examples intrauterine membrane endometriosis associated with infertility patients as the research object,were performed laparoscopic surgery,using simple sampling method randomly divided into A,B,C three groups,each group of 41 cases.Group A was given subcutaneously injected with bright C gonadorelin acetate microspheres,Group B with oral pregnant three ketene capsule,group C was given expected treatment.Behavior period for 2 years follow-up,the observe comparing three groups of patients with recurrence after treatment and pregnancy outcomes.Results（ 1） After 2years＇ follow-up,the total pregnancy rate of group A was 78.05%,compared with 73.17% of group B had no obvious difference（ P〉0.05）; two groups of A,B clear difference compared with 43.90% of group C（ P〈0.05）;（ 2） Followed up for 2 years,A,B two groups each recurrence of 3 cases,the recurrence rate was 7.32%,comparison had no statistical significance（ P〉0.05）; Clear difference compared with 26.83% of group C（ P〉0.05）; periodⅠ of 53 patients total pregnancy rate followe-up for 2 years was 64.15%,compared with 61.91%of Ⅱ period,had no obvious difference（ P〉0.05）; period Ⅳ of 7 patients were 2 cases of pregnancy within 2years,the pregnancy rate was 28.57%,compared with 47.62% of Ⅲ period had no significant difference（ P〉0.05）,compared with Ⅰ,Ⅱ period in patients had significant difference（ P〈0.05）.Conclusions The curative effect of giving subcutaneously gonadorelin acetate microspheres injection to patients with endometriosis associated with infertility is good,which can effectively increase the rate of patients with pregnancy,worth of clinical promotion.
