摘要
目的比较分析子宫内膜异位症伴不孕患者腹腔镜术后不同治疗方法对妊娠率的影响。方法选择住院治疗子宫内膜异位症合并不孕的患者176例,排除双侧输卵管完全性阻塞及男性不育者。将Ⅰ、Ⅱ期患者作为A组(期待治疗组),Ⅲ、Ⅳ期患者随机分B组(孕三烯酮+米非司酮组)和C组(亮丙瑞林组),将随访结果进行统计分析。结果 A、B、C组总妊娠率分别为40.4%、62.8%、61.5%。期待治疗组(A组)较其他两组妊娠率低(P<0.05)。术后1年内妊娠者67例;第2年内妊娠者19例,其中Ⅲ、Ⅳ期患者9例;2年后妊娠仅3例,2年后Ⅲ、Ⅳ期患者无1例受孕。结论腹腔镜手术后加用抗子宫内膜异位的激素类药物治疗可提高妊娠率。
Objective To compare and analyze the different treatments and the influence of the pregnancy rate in infertile patients with endometriosis treated by laparoseopic operation. Methods A total of 176 cases of endometriosis merged with infertility were chosen and treated by laparoscopic operation in our hospitial, while patients of double - side perfectibility fallopian tube emphraxis or male infertility were eliminated. Patients in phase Ⅰ、Ⅱwere Group A （ Expecting Treatment Group） ; patients in phase Ⅲ、Ⅳ were divided into Group B （Gestrinone combined with Mifepristone Group） and Group C （Leuprorilin Group） stochastically, and the result was statistically analyzed. Results The chief pregnancy rate of Group A, B, C was 40.4 % , 62.8 % , 61.5 % respectively. The pregnancy rate of Group A was lower than that of the other two groups （ P 〈 0.05 ）. 67 women were in gestation 1 year after operation. Among 19 gestation women the next year, there were 9 cases patients in phase Ⅲ、Ⅳ Among only 3 gestation women after 2 years, no patient in phase Ⅲ、Ⅳ became pregnant. Conclusion Laparoscopic operation combined with hormones of contradicting endometriosis may increase the pregnancy rate of infertility with endometriosis.
