摘要
目的评价智能导引装置(ROBIO)辅助CT引导下肺穿刺活检术的可行性和安全性。方法对20例患者行ROBIO辅助CT引导下肺穿刺活检术,记录操作时间、定位时间、穿刺次数、扫描次数、辐射剂量、瞄准精度以及并发症情况。结果肺活检平均操作时间为(32±8.3)min(24-50min),平均定位时间为(13.8±4.1)min(8-27min)。20例患者成功19例,共穿刺了20次,平均穿刺次数为(1.1±0.2)次(1-2次)。平均扫描次数为(3.6±0.8)次(3-6次),剂量长度乘积(DLP)均值为(570.5±167.8)m Gy·cm(411.5-1117.5 m Gy·cm)。20次穿刺定位中,16次瞄准精度〈5 mm,3次为5-10 mm,1次为12 mm,穿刺针瞄准精度均值为(3.7±2.2)mm(1-12mm)。20例中气胸2例,血胸1例,无严重并发症发生。结论 ROBIO辅助CT引导下肺穿刺活检术定位时间短,穿刺精确度高,并发症发生率低,是一种导引肺穿刺定位安全、有效的方法。
Objective To estimate the feasibility and safety of CT-guided percutaneous lung biopsy by using an intelligent guiding device called ROBIO. Methods A total of 20 patients underwent ROBIO-assisted CT-guided percutaneous lung biopsy. The procedure time,localization time,puncturing times,scanning times,radiation dose( Dose-length product,DLP),aiming accuracy,and complications were recorded. The results were analyzed. Results The mean procedure time was 32 ± 8. 3min( range 24- 50 min),localization time was 13. 8 ± 4. 1 min( range 8- 23 min),and the mean puncturing times were 1. 1 ± 0. 2( range 1- 2),scanning times were 3. 6 ± 0. 8( range 3- 6),mean DLP value was 570.5 ± 167. 8( range 411. 5- 1117. 5) m Gy · cm. Biopsy was performed successfully in 19 of 20 patients,and totally 20 times of puncturing were carried out in 19 patients. Needle aiming accuracy was less than 5 mm in 16 punctures,5- 10 mm in 3 punctures and 12 mm in one puncture. Two patients developed pneumothorax and one developed haemothorax,and no serious puncturing-related complications occurred. Conclusion As it has short localization time,high aiming accuracy and low incidence of complications,ROBIO-assisted CT-guided percutaneous lung biopsy is an effective and safe technique.
