摘要
目的探讨浸润性乳腺癌的超声影像学特点以及其临床病理特性与超声影像学观察指标之间的关系,以提高超声影像诊断技术对浸润性乳腺癌的诊断价值。方法回顾性分析探讨100例浸润性乳腺癌的超声影像学特点,并分析其超声声像图特征和免疫组化指标(ER、PR、HER-2)之间的关系。结果浸润性乳腺癌超声影像特征为纵横比大于1,不规则的形态,边缘多有毛刺征,内部回声较低,可见后方回声衰减,伴有微钙化。浸润性乳腺癌中肿块纵横比>1、毛刺征、周围晕环征、微钙化均与ER阳性表达相关,毛刺征、周围晕环征与PR阳性表达相关;肿块纵横比>1、微钙化与HER-2阳性表达相关,肿块形态、后方回声变化、血流均与ER、PR、HER-2表达无显著相关性。结论浸润性乳腺癌超声表现有其相关的病理学基础,某些超声特征与ER、PR和HER-2的表达有一定的相关性。
Objective To study the ultrasonic imaging characteristics of infiltrating breast cancer and the relationship between the ultrasonic imaging observation index and its clinical pathological characteristics ,in order to improve the ultrasonic imaging diagnostic technique for the diagnosis of infiltrating breast cancer .Method 100 cases of infiltrating breast cancer of the ultra-sonic imaging characteristics ,and the relationship between the ultrasonographic features with immunohistochemical index （ER , PR ,HER-2） were retrospectively analyzed .Results The ultrasound imaging characteristics was aspect ratio greater than 1 ,ir-regular shape ,many edge burr ,low echogenicity ,visible rear echo attenuation and micro calcification in infiltrating breast canc-er .Infiltrating breast cancer tumors aspect ratio 〉1 ,burr ,halo around the ring sign ,microcalcification were related to the ER positive expression .burr ,halo around the ring sign were associated with PR positive expression ,tumor aspect ratio 〉1 ,micro-calcification were related to the HER-2 positive expression ,tumor shape ,rear echo change ,blood flow and ER ,PR ,HER-2 of expression had no significant correlation .Conclusion Sonographic findings of infiltrating breast cancer had its related pathology foundation ,certain sonographic features and expression of ER and PR and HER-2 had certain relevance .
