摘要
目的 :初步评价高强度聚焦超声 (HIFU)临床治疗肝癌的疗效及安全性 ,探寻肝癌无创性治疗的新方法。方法 :应用HIFU治疗原发性肝癌 86例 ,转移性肝癌 7例 ,分别给予完全覆盖 5 2例 ,部分覆盖 4 1例。通过症状、主要脏器功能测定、组织病理学、彩色多普勒超声、MRI或CT、血清AFP水平等指标观察患者的治疗效果及临床安全性。结果 :显效 2 8例 (30 1% ) ,有效 5 3例(5 7% ) ,无效 12例 (12 9% ) ,总有效率 87%。治疗后患者临床症状减轻 ,血清AFP水平下降 ,病灶缩小 ,血供减少或消失 ,无明显局部并发症发生 ,部分患者有治疗区皮肤损伤 ,均可自行愈合 ,小部分患者术后有发热及轻微腹痛 ,2～ 3日后消失。组织病理学发现HIFU治疗区肿瘤组织呈凝固性坏死。结论 :HIFU对于肝癌的治疗是有效的 。
Objective: To evaluate the efficacy and safety of high intensity focused ultrasound (HIFU) in the treatment of liver cancer Methods: 93 cases presenting with liver carcinoma and metastasis were treatment with HIFU, 52 cases had their tumor tissue completely destroyed, 41 cases had their tumor tissue partly sonicated Evaluate the efficacy and safety was made on the basis of main organs functional test, histopathological changes, variations in color Doppler sonography and CT or MRI, tumor size, serum AFP Results: Symptom alleviation, serum AFP decrease, tumor shrinkage, and reduction of blood supply for tumor were observed in the 81 cases(87%) after HIFU sonication No local complication was seen, and liver and kidney functions remained normal Conclusion: The use of HIFU in the treatment of liver cancer is effective and feasible This new technique will offer a considerable noninvasive therapy for local treatment of liver cancer
