摘要
目的 探讨经颈外静脉插管至深静脉行血液透析的方法及其优点和护理。方法 47例血液透析患者 ,使用AR ROW抗感染大口径双腔中心静脉导管 ,在充盈的颈外静脉上方穿刺 ,利用导丝进入颈外静脉与锁骨下静脉交角处到上腔静脉行血液透析。结果 45例穿刺成功 ,2例失败。一次成功率为 91.5 % ,操作时间 5～ 15min ,血流量可达 2 0 0～ 3 0 0ml/min ,血液透析效果与常规穿刺法相似 ,无 1例感染及并发症发现。结论 经颈外静脉插管术操作简单 ,危险性小 ,置管后可免除每次血液透析穿刺时的痛苦。护理上应重视肝素封管法。
ObjectiveTo explore the nursing characteristics of hemodialysis with anti infective double lumen catheter inserted into external jugular vein through central vein.Methods Hemodialysis were performed for 47 patients with renal failure by way of inserting anti infective double lumen catheter through external jugular vein to central vein using Raulerson syringe. ResultsPunctuation and insertion of catheter in 45 cases was successful. The success rate at one time reached 91.3%. Time consumption for each case was 5～15 min. The blood flew at the speed of 200～300 ml/min. No infection and complications were found.ConclusionThe punctuation and insertion of catheter from external jugular vein to central vein is safe and less dangerous. Indwelling catheter can free the patients from pains at punctuating for hemodialysis each time. In nursing, sealing the double tube catheter with heparin fluid is key in prevention of blood coagulation.
