深低温冷冻同种异体气管游离移植的实验研究 被引量:5

摘要 目的 研究犬气管经液氮深低温冷冻削减抗原后 ,用于同种异体移植 (不用免疫抑制剂 )的可行性。 方法 实验犬 17只 ,通过纤维支气管镜检查、粘膜血流量的测定、病理学检查、血管造影以及存活率和通畅度的计算 ,分别研究以单侧胸头肌瓣和双侧胸骨舌骨肌 胸骨甲状肌联合瓣包裹自体移植气管、未冷冻以及冷冻 6周的同种异体移植气管术后的存活情况。 结果 冷冻 6周的同种异体气管移植后与自体移植的气管无明显差别 ,未见有单核细胞浸润 ,可长期存活 ,病理评分分别为 2 2 2± 0 37和 1 33± 0 2 8。未冷冻的同种异体气管移植后第 1周时 ,粘膜外观以及粘膜血流量与冷冻 6周的同种异体移植气管相同 ,但在第 2周时粘膜呈现灰黑色 ,粘膜血流量低于第 1周时的测定值。于术后 2~ 4周移植气管坏死。显微镜下见移植气管内有大量单核细胞浸润 ,病理评分为19 0 0± 0 72。 结论 气管经深低温冷冻后可以削减其抗原性 ,同种异体移植后不发生免疫排斥反应 。 Objective To determine the feasibility of dog allotransplant with decreased antigenicity by cryopreservation. Methods Fiberoptic bronchoscopy, histopathology and microangiography were used to measure tracheal mucosal blood flow and calculate the survival rate and patency of autograft tracheas and allograft tracheas without or with cryopreservation for 6 weeks in one sided sternocephalic muscle flap and two sided sternohyoid sternothyroid muscle flap. Results No significant difference was observed between autograft and allograft cryopreservated for 6 weeks. No infiltration of mononuclear cells was found and pathologic evaluation score was 2 22±0 37 and 1 33±0 28 respectively. The allograft could survive for a long time. Both mucous membrane appearance and mucosal blood flow of the allografts without cryopreservation were the same at the 1st week after transplantation as those of the allografts cryopreservated for 6 weeks, but the mucous membrane was in black and partial cartilaginous rings were exposed at the 2nd week after transplantation. Necrosis occurred shortly after transplantation and infiltration of mononuclear cells in allograft was observed under light microscope. The pathologic evaluation score was 19 00±0 72. Conclusion The antigenicity of the tracheal allografts can be decreased by cryopreservation. No allograft rejection takes place in these allotransplantations and the allografts can survive for a long time.
