AIM: To study the mechanism and treatment of severebiliary complications arising from hepatic artery embolization(HAE).METHODS: Of seven cases of intra- and extrahepatic biliarydamage resulting from hepatic artery embolization reportedsince 1987, 6 suffered from hepatic haemangioma, the othercase was due to injection of TH compound into the hepaticartery during operation. The hepatic artery was injected withethanol so as to evaluate the liver damage in experimentalrats.RESULTS: All the cases were found to have destructivedamage of intra- and extrahepatic bile duct at the hilum withbiliary hepetocirrhosis. Experimental results revealednecrosis of the liver parenchyma, especially around theportal tract and obliteration of intrahepatic bile duct.CONCLUSIONS: To prevent the severe biliary comlications ofHAE, the use of HAE for hepatic haemangioma which waswidely practiced in China, should be re-evaluated. Hepaticarterial embolization of hepatic haemangioma may result insevere destructive biliary damages and its indiscriminate useshould be prohibited.
