摘要
目的 观察不同的麻醉方法对上腹部手术患者血浆 6 酮 前列腺素F1α( 6 keto PGF1α ,PGF1α)和血栓素B2 (TXB2 )的影响。方法 择期上腹部手术患者 2 0例 ,随机分两组 :Ⅰ组为硬膜外麻醉组 ;Ⅱ组为全身麻醉组。于麻醉前、诱导后、手术 10min、手术 5 0min及术毕采静脉血 ,用放射免疫法测定血浆中PGF1α和TXB2 的含量。结果 ( 1)PGF1α在Ⅰ组麻醉后升高 ,术毕明显高于术前 (P <0 .0 5 ) ;Ⅱ组诱导后下降 ,然后渐升 ,术毕达高峰 (P <0 .0 5 )。 ( 2 )TXB2 在Ⅰ组麻醉后均明显降低 (均为P <0 .0 1) ;Ⅱ组诱导后显著降低 (P <0 .0 1) ,但术毕却明显升高 (P <0 .0 5 )。 ( 3)PGF1α与TXB2 的比值 (K/T值 )在Ⅰ组麻醉后均显著升高 (均为P <0 .0 1) ,而Ⅱ组仅见诱导后显著增高。
Objective To observe the effects of different anesthesia methods on the levels of plasma prostaglandin F1α(PGF1α) and TXB 2 in the patients undergoing upper abdominal surgery.Methods Twenty patients,undergoing elective upper abdominal surgery,were randomly divided into two groups.Group Ⅰ was given T 8?9 epidural anesthesia.General anesthesia was used in group Ⅱ.Venous blood samples were taken at five time points:before anesthesia,after induction,at 10?min and 50?min after incision and at the end of surgery.The contents of PGF1α and TXB 2 were measured by radioimmunoassay.Results (1)The levels of plasma PGF1α were increased after anesthesia and obviously higher than that of pre operation at the end of operation in group Ⅰ( P <0.05).The levels of PGF1α were significantly decreased after induction,then increased gradually,and reached to the peak at the end of operation in group Ⅱ( P < 0.05 ).(2)The levels of plasma TXB 2 were significantly decreased after anesthesia in groupⅠ(all, P <0.01).The levels of TXB 2 were significantly decreased after induction( P <0.01) and significantly increased at the end of operation in group Ⅱ( P <0.05).(3)The ratio of PGF1α to TXB 2 was significantly increased after anesthesia in group Ⅰ(all, P <0.01),but was significantly increased after induction in group Ⅱ.Conclusion The epidural anesthesia is better than general anesthesia to attenuate early stress response after operation.
