摘要
目的探讨皮肤软组织感染患者金黄色葡萄球菌感染状况及对常用抗菌药物的耐药性,为临床抗菌药物的选择提供参考。方法选取2016年1月-2017年10月于医院治疗的皮肤软组织感染患者200例为研究对象,采集局部分泌物行病原菌培养,以Vitek-32全自动微生物分析仪进行菌种鉴定及药物敏感性试验,分析耐甲氧西林金黄色葡萄球菌(methicillin-resistant Staphylococcus aureus,MRSA)的检出情况。结果 200例皮肤软组织感染患者临床送检标本均检出病原菌,其中112例为金黄色葡萄球菌感染,占56.00%(112/200),112例金黄色葡萄球菌感染患者中82例为MRSA感染,占73.21%(82/112);皮肤脓肿、疖、痈感染,MRSA检出率为65.91%,检出率最高;皮肤软组织不同感染类型MRSA检出率比较差异有统计学意义(χ^2=41.303,P<0.001),外科MRSA检出率最高为68.97%,不同科室MRSA检出率比较差异有统计学意义(χ^2=49.826,P<0.001);82例MRSA感染患者中1例利奈唑胺耐药、1例万古霉素耐药。结论金黄色葡萄球菌是引起皮肤软组织感染的主要病原菌,其中MRSA检出率较高,对利奈唑胺和万古霉素耐药率较低。
OBJECTIVE To explore the prevalence of Staphylococcus aureus infection in patients with skin and soft tissue infection and analyze the drug resistance to the commonly used antibiotics so as to provide guidance for clinical use of antibiotics.METHODS A total of 200 patients with skin and soft tissue infection who were treated in hospitals from Jan 2016 to Oct 2017 were recruited as the study objects,the local secretions were collected for culture of pathogens,the isolated pathogens were identified by using Vitek-32 automatic microorganism analyzer,the drug susceptibility testing was performed,and the isolation rate of methicillin-resistant S.aureus(MRSA)was analyzed.RESULTS The pathogens were isolated from the clinical submitted specimens of the 200 patients with skin and soft tissue infection,112 patients had S.aureus infection,accounting for 56.00%(112/200);totally 82 of 112 patients with S.aureus infection had MRSA infection,accounting for 73.21%(82/112).The isolation rate of MRSA was 65.91%among the pathogens with skin abscess,furuncle,carbuncle infection;there was significant difference in the isolation rate of MRSA among the patients with different types of skin and soft tissue infection(χ^2=41.303,P<0.001).The isolation rate of MRSA was the highest in surgical departments,and there was significant difference in the isolation rate of MRSA among the departments(χ^2=49.826,P<0.001).Among the 82 patients with MRSA infection,1 was resistant to linezolid,and 1 was resistant to vancomycin.CONCLUSION S.aureus is the predominant species of pathogen causing the skin and soft tissue infection,the isolation rate of MRSA is high,and the drug resistance rates to linezolid and vancomycin are low.
