摘要
目的 :通过观察离断兔耳 ,在不同时限再植后末梢血流、组织细胞变化 ,探讨复合组织游离移植缺血时限及其与预后的关系。方法 :采用兔耳离断再植模型 ,分别观察离断 8、12、16、2 0、2 4、>2 4h再植后 ,末梢毛细血管血流的变化 ,以及兔耳最终存活后组织细胞的变化。结果 :离断 8h组末梢毛细血管灌注良好 ,细胞形态无明显变化 ;12h组 ,末梢灌流稍差 ,但断耳最终存活 ;>16h组仅有大血管灌流 ,且慢 ,呈摆流 ,毛细血管内呈淤血或无灌流状态 ,断耳最终 4/32存活。病检显示 ,12h以后各组 ,组织细胞出现逐渐加重核固缩现象。结论 :兔耳离断再植的最佳时限应该 <8h。结合临床 ,与兔耳组织类似的手指 ,再植或异体组织移植的时限也最好 <8h ;而耐缺氧较差的其他复合组织移植时或出现危象后再探查的时限应该更短 ,否则即使存活 。
Objective:To analyze the best time limitation of the severd fingers.Methods: 25 rabbits were divided into 6 groups:A,B,C,D,E.That's 8,12,16,20,24 hours after the ears' blood flow were temporarily cut.The capillary filling and the cells'changing of the distal part of the ears were observed.Results: The result shows that in the group A which the blood flow were refilled after 8 hours cut,the capillaries of the distal part of the ears filled excellently,and the cells stay unchanged;In group B, capillary filling was not so good,but the ears survived;otherwise the cartilage cells showed lowgrade languished.And in the rest groups C,D,E,the filling of capillary were poor,4/32 of the animals'ear survived finally,the histology study showed that the cells in the distal part of the ears were remarkably languished.Conclusions: The best limitation time of the severd ear of rabbits should be shorter than 8 hours.Clinically,we suggest that the time limitation of the severd finger or other similar compound tissue transplantatiaon in which the time limitation should also be shorter than 8 hours.
