摘要
目的 探讨脐血筛选标准的制定及在脐血库建立中的意义。方法 按脐血采集量、总有核细胞数、细胞活率、病原学感染、有无凝块等作标准进行脐血采集筛选 ,分析筛选后库存脐血的质量。结果 1998年 6月至 1999年 12月共采脐血 16 2 5例 ,合格 899例 ,占 5 5 .3%。其中 879例分离冻存 ,占 5 4.1% ;不合格 72 6例 ,占 44 .7% ,不合格原因主要为 :脐血采集量少、有凝块、细胞活率低、超时送检、总有核细胞数低等。 879例库存脐血总有核细胞数平均为 1.2 36× 10 9,平均有效移植体重为33.4kg ,88.9%的库存脐血能满足体重为 2 0kg以上的患者移植。脐血细菌培养共检出 4例阳性 ,巨细胞病毒抗体 3例阳性 ,人类免疫缺陷病毒抗体 (HIV 1/ 2Ab)、EB病毒抗体 (EBV IgA)、乙型肝炎病毒表面抗原 (HBsAg)、丙型肝炎病毒抗体 (HCV Ab)均为阴性。 9例脐血移植中 6例获得植入。结论 脐血筛选标准的制定可节省建库费用 ,提高库存脐血质量 ,使库存脐血均符合临床移植要求 。
ObjectiveTo study the establishment and significance of cord blood screening standard in cord blood bank. MethodsCord blood were screened according to cord blood collected volume, total nucleated cells, cell viability, pathogenic infection, clot and so on. The quality of the bank blood after screening was analyzed. Results 1 625 units of cord blood were collected from June 1998 to Dec. 1999. 899 units were qualified ( 55.3%). 879 units were frozen ( 54.1%). The most factors of disqualification included low collected volume, clot, low cell viability, overtime deliver, low nucleated cells and so on. The average nucleated cells of 879 units of frozen cord blood were 1.236×10 9 and the average weight of available transplantation 33.4?kg. 88.9% of frozen cord blood was suitable to the transplant for the patients with weight over 20?kg. Four cases were positive for germiculture and 4 positive for cytomegalovirus antibody. All the cases were negative for human immunodeficiency virus (HIV) antibodyk, Epstein Barr virus (EBV) antibody, hepatitis B surface antigen (HBsAg) and hepatitis C virus (HCV) antibody. Six of 9 cases of transplantation were engrafted. ConclusionsCord blood screening can economize the expense of cord blood bank and improve quality of the frozen cord blood. It is important to screen cord blood in the establishment of cord blood bank.
