摘要
目的探讨总有核细胞和CD34+细胞数量在脐血移植中选择合适脐血的预示作用。方法分析不同总有核细胞数(TNC)和CD34+细胞数输入量、供受者人类白细胞抗原(HLA)不相合数间脐血移植效果的差异,评价TNC和CD34+细胞数的作用。结果 89例白血病患者植入75例,植入率为84.3%。中性粒细胞(ANC)≥0.5×109/L、血小板≥20×109/L、血小板≥50×109/L的时间分别为17d、34d和46d。TNC输入量与移植植入率呈正相关,与生存率无关。CD34+细胞输入量与移植植入率和生存率有正相关性,供受者HLA配型不相合位点数差异对移植后的植入率和生存率无影响。结论脐血是一种替代骨髓造血干细胞应用于白血病患者移植的较好来源。TNC和CD34+细胞数输入量对移植效果存在正性影响作用,是脐血移植中选择合适脐血的重要参考指标。
Objective To discuss the predictive effect of infuse total nucleated cells (TNC) and absolute CD34+ceIls number for cord blood selection in unrelated cord blood transplantation(UCBT). Methods We analyzed the difference of cord blood transplantation outcomes in different infused TNC and absolute CD34+cells number and HLA disparity and discussed the effect of TNC and absolute CD34+cells number in cord blood selection before UCBT. Results Seventy-five patients were engraft in 89 UCBT and the incidence of engraftment was 84.3%. The average time to achieve neutrophil reconstitution at 0.5xl&/L, platelet reconstitution at 20x109/L and platelet reconstitution at 50x109/L werelT, 34 and 46 days, respectively. Infused TNC number was positively influence engraftment rate and had no influence to survival rate after UCBT. Infused absolute CD34+ cell number was positively influence the engraftment rate and survival rate, while HLA disparity had no influence to the engraftment rate and survival rate. Conclusion Cord blood as a source of hematopoietic stem cells, is a reasonable option for transplantation in patients with leukemia. The infused number of TNC and absolute CD34~cell number are positively influence the transplantation outcomes and better reference indicator in the selection process of cord blood units for transplant.
