运用双能X线骨密度仪观察下肢延长过程中新生骨痂BMD的变化 被引量:1

摘要 为探讨运用双能X线骨密度仪 (DEXA)动态观察下肢股骨或胫骨骨痂牵拉延长矿化过程的临床价值 ,本文对 2 7例下肢延长术患者在术后牵引期及骨痂生长期股骨或胫骨新生骨痂的骨密度 (BMD)进行检测 ,并将截骨间隙处中心及边缘骨质分为 7个兴趣区 (ROI)加以分析。结果显示 ,在牵引的第 4周 ,新生骨痂达到峰值 ;而牵引第 8周 (术侧肢体处于最大牵引长度时 ) ,BMD降至最低。骨延长停止后 ,BMD又逐渐上升 ,直至术肢外固定器去除。此外 ,靠近下肢延长固定器侧兴趣区内的骨痂新生量低于对侧。表明DEXA能对下肢延长期间新生骨痂进行量化测定 ,有助于临床监测肢体延长过程中骨愈合的状况。 To explore the clinical applicable values of Dual Energy X-ray Absorption (DEXA) on dynamically monitoring the mineralization of the neoformative callus during the prolonged period of femur or tibia, the BMD of the neoformative callus in fe mur or tibia of 27 cases made lengthening operation was measured during the peri od of traction and porotic stage, and 7 regions of interest (ROI) were selected out in the osteotomized region. The results showed that at the 4th week after th e traction, the BMD of the neoformative callus achieved the peak value, while at the 8th week after the traction (the extremity arrived to the biggest length), the BMD dropped to the minimum value, but with the stoppage of the bone lengthen ing, the BMD was gradually going up until the external fixator was taken out. Fu rthermore the quantity of the neoformative callus in the ROI near the lengthenin g fixator in the lower extremity was lower than that of the opposite side. Sugge sting that DEXA can measure the BMD of the neoformative callus during the length ening period of lower extremity quantitatively; it also benefits the monitoring the union of the lengthening period of lower extremity clinically.
