摘要
目的 :为阐明延长诱发关节软骨损伤的力学机制提供参考。方法 :以新鲜离体兔胫骨干骺端截骨微型半环槽式外固定器不同增量牵伸延长为实验模型 ,利用压力传感器测定延长后即刻、3min、5min时膝关节接触压力值变化 ,同时测定骨延长幅度。结果 :( 1)关节接触压随延长幅度加大而加大 ,当骨延长幅度低于 6%时 ,接触压增加趋势尚缓 ,但此后则上升趋势明显加快 ,尤以 1.5mm/次组为甚 ,0 .5及 1mm/次组上升趋势相似。 ( 2 )延长后即刻、3min、5min3个时相点的关节接触压无显著性差别 (P >0 .0 5 )。 ( 3 )固定器牵伸长度与骨延长幅度呈线性关系 ,但后者具有滞后性。结论 :肢体延长可导致关节接触压增大 ,并与延长速度及幅度成正比 ,因而有诱发关节软骨高应力损伤的可能。
Objective: To offer a reference for elucidation of biomechanic mechanism on articular cartilage injury during tibial lengthening. Methods:The changes of contact pressure of knee was observed at immediate,3 and 5 minutes after a distraction with the force compression load cell by a cadaver tibial metaphysis lengthening model fixed with mini half ring sulcated external fixator.3 kinds of distraction rates,0.5,1.0 and 1.5mm per time,were adopted.The bone lengthening length was determined also.Results:The contact pressure of knee increased with bone lengthening and its rising tendency was faster after 6 percent of tibial length.The pressure in 1.5mm group was larger than that in 0.5 or 1.0mm group.There was no significant difference in pressure after bone lengthening and 3 or 5 minutes after a distraction( P> 0.05)。Conclusion:Bone lengthening might be one of the important factors for high stress damage of articular cartilage through increment of contact pressure of joint,which correlated positive ratio with speed and range of lengthening.
