^(18)F-FDG PET脑显像对小儿原发性癫痫病灶定位诊断的研究

摘要 目的 评价18F -FDGPET脑显像对小儿原发性癫痫定性和病灶定位的诊断价值。方法 对 3 2例CT或MRI检查正常的小儿原发性癫痫患者进行18F -FDGPET脑显像 ,2 9例发作间期、3例发作期显像 ,并同步进行EEG检查 ,对两种检查结果进行比较。结果 发作间期 2 9例患者PET显像 2 5例异常 ,发作期 3例患者均异常 ,总阳性率 88% ;EEG检查异常的 2 0例患儿中与PET异常部位完全或基本符合率为 70 %。结论 18F -FDGPET脑显像是诊断小儿原发性癫痫病灶的有效方法 ,灵敏度优于EEG ,但PET所示异常范围并非都是致痫灶 ,尚需与EEG结合分析 。 Objective To evaluate the diagnostic value of 18 F-FDG PET imaging in localizing epileptic foci in children with primary epilepsia. Methods 18 F-FDG PET brain imaging was performed in 32 children with primary epilepsia and normal CT or MRI. 29 were interictal and 3 were ictal. Electroencephalogram (EEG) was recored simultaneously. All results were compared with each other. Results 25 of 29 interictal patients had abnormalities on PET imaging, all 3 ictal patients had abnormal PET. The total positive rate was 87.5%. The epileptic foci localized by EEG(20 abnormalities) was 70% in concordance with those by PET. Conclusions 18 F-FDG PET brain imaging is a sensitive and effective method for epileptic foci localization in children. It may have some advantages over EEG. However, abnormal PET areas may not be epileptic foci. A combination of PET and EEG will improve the positive rate and accuracy for localization.
