职业性氟暴露对钙调节激素及肌电生理活动、骨密度影响的研究

摘要 目的 :探讨职业性氟暴露对降钙素、甲状旁腺素、骨密度含量、肌电生理活动影响。方法 :选择某铝厂电解铝工人 6 0名为观察组 ,年龄 38~ 48岁 ,平均 40 77岁 ,氟作业工龄平均 19 5 2年 ,范围 5~ 2 2年 ;选择在年龄、生活习惯、居住环境、工龄等方面与观察组有可比性的同企业机械厂男性工人 5 0名为对照组 ,进行 ( 1)职业史、既往史、自觉症状、内科体检等 ;( 2 )骨骼X 拍片 (观察组 ) ;( 3)周围神经肌电传导速度测定 ;( 4 )骨密度测定 ;( 5 )车间空气氟化物测定 ;( 6 )实验室 :CT、PTH、F-、Ca2 + 、Mg2 + 、Cu2 + 、Zn2 + 、Se2 + 、AKP、SOD、GSH Px、CAT、IgG、IgA、IgM测定等。结果 :在临床表现方面接氟工人主要以腰背痛 ( 6 3 3 % )、四肢关节痛 ( 6 3 3 % )、胸闷 ( 5 3 1% )、乏力 ( 91% )、脱发 ( 70 5 % )等症状明显 (P <0 0 1) ;接氟工人胫、腓、尺、桡及骨盆腰椎X线摄片显示 ,76 %骨密度增高 ,44%骨小梁增粗 ,80 %骨膜增生 ;测定的 16项生化指标中 ,尿F-、血F-、CT、PTH、AKP、GSH Px与对照组相比 ,差异有显著性 ;血清F-与GSH Px呈明显的负相关、尿F-与CAT有正相关 ,观察组CT与PTH间则有明显的正相关 (P <0 0 1) ;骨密度的测量可见整体趋势小于 2 0年电解工龄者较大于 2 Objective: To study the effects of coccupational exposed fluoride on Calcitonin (CT) and parathyrid hormone (PTH) and muscular electric activities and bone mineral densety(BMD).Methods: Choose sixty electrolysis aluminum workers aging from 38~48 in an aluminum factory as the trial group.Their average age should be 40.77 years and they should at least work in this field for 19.52 years on the average.The scope is 5 22 years.Choose fifty male workers at a machinery plant of the same enterprise as the contrast set who can be compared with the trial group in age,living habits,living environment and length of service.And carry out the following determinations.(1)Occupation history,previous history,self felt symptoms and medicine examination and so on.(2)Bone X rays(trial group).(3)Periphery nerves muscle electricity conduction speed.(4)Bone mineral density.(5)Fluorine content in the air in workshops.(6)Measuring:Calcitonin(CT),Parathyrid hormone (PTH),F -,Ca 2+ ,Mg 2+ ,Cu 2+ ,Zn 2+ ,Se 2+ ,alkaline phosphatase (AKP),superoxide dismutase(SOD),glutathione peroxidase(GSH Px),catalase(CAT),IgG,IgA and IgM in the lab.Results: At the aspect of the clinical manifestation,workers in contact with fluorine have evident symptoms pains in the back,waist and limbs joins (63.3%),asthenia(91%),lose one's hair(70.5%),were significantly higher control group( P <0 01);Their X rays into tibia,fibula,ulna,radius,pelvis and lumbar vertebra show a lot.①76% of them get higher bone density.②44% of them get thicker small bone beam.③80% of them have periosteum hyperplasia;In 16 biochemistry indexes ascertaining by measuring or surveying,the trial group has notable differences in the urine F -,serum F -,CT,PTH,AKP and GSH px compared with the control group.The serum F - and GSH Px assume clear negative relation.The urine F - and CAT have positive relation.The CT and PTH of the observation group has clear positive relation ( P <0 01);the measurement of bone density shows that the BMD of workers who serve in this field less than 20 years in higher than those whose length of service is longer than 20 years.The muscular electric activities shows that the peripheral nerves feeling conduction speed of the observation group slows down and the latent period of the limbs feeling inducing electric potential prolong.Conclusion: The excessive fluorine leads to the change of adjusting calcium hormone through the influence on calcium metabolize.At the same time,its injury to thyroid gland and parathyroid gland also in fluences the secretion of the adjusting calcium hormone.The change of the hormone of calcium adjusting results in the disorder of bone metabolize and in this way the pathology change of bones appears; the nerve muscle electricity physiology activities and sensory nerve system change and AKP and GSH Px can be regarded as the healthy guardianship guideline of occupation.
