摘要
目的 :运用MRIcine相位对比法研究正常志愿者的脑脊液流动。材料和方法 :采用MRIcinePC对40例正常志愿者颅内及椎管脑脊液循环进行研究 ,男29例 ,女11例 ,年龄7~76岁。对其中31例作了中脑导水管、枕大孔和C2 水平的脑脊液流动方向、流速和流量的研究 ,9例测定了T6 及L1 水平的脑脊液流动方向。结果 :(1)心房收缩期和心室收缩期 ,脑脊液自第三脑室向下流入第四脑室 ;心室舒张期 ,则脑脊液流动方向相反。椎管内心脏收缩期和舒张期脑脊液流动方向与中脑导水管基本相同 ,即收缩期脑脊液向下流动 ,而舒张期脑脊液向上流动。脑脊液在椎管内的流动方向蜿蜒曲折 ,颈和腰段椎管内能测得明显脑脊液流动和确定其流向的部位在椎管的前部 ,而胸段则在椎管的后部。(2)每个心动周期中脑导水管脑脊液向下峰速和向上峰速分别为15.6±6.58mm/s和15.92±6.79mm/s ,中脑导水管的向下流量及向上流量分别为0.547±0.208ml/s及0.538±0.208ml/s ,净向下流量为0.009±0.008ml/s ;枕大孔脑脊液向下峰速和向上峰速分别为2.47±0.98mm/s和2.94±1.34mm/s ,向下流量及向上流量分别为0.534±0.249ml/s及0.530±0.250ml/s ,净流量为0.004±0.002ml/s ;C2 层面脑脊液向下峰速和向上峰速分别为4.72±1.86mm/s和4.89±1.78mm/s 。
Purpose:To study the flow of cerebrospinal fluid(CSF) in normal volunteers with MRI in phase contrast cine mode(MRI cine PC).Materials and Method:A 1.5T GE Horizon Signa MRI was used in this study.Forty normal volunteers,7~76 years old,29 males and 11 females,were investigated.In 31 of them the flow of CSF at the levels of aqueduct,foramen magnum and C2 was examined to detect the direction,velocity and magnitude of the CSF flow during various periods of the cardiac cycle.The direction of CSF flow at T6 and L1 level was tested in the rest 9 normal volunteers.Results:(1)The speed and direction of CSF flow changed during the cardiac cycle in normal volunteers.The direction of flow was from the cranial cavity towards the spinal canal during auricular and ventricular systolic period,and was reversid during ventricular diastolic period.The direction of CSF flow in the spinal canal followed the same rule as in the aqueduct,flowing down-and upward in systolic and diastolic period respectively.The flow was mainly found in the anterior part of the cervical and lumbar spinal canal,and in the posterior part of thoracic spinal canal.(2)In the aqueduct of normal volunteers the outflow and inflow peak velocity were 15.6±6.58mm/s and 15.92±6.79mm/s respectively,the volumes of the outflow and inflow of CSF were 0.547±0.208ml/s and 0.538±0.208ml/s respectively,and the net flow volume of CSF was 0.009±0.008ml/s from the 3rd ventricle to the 4th ventricle.At the level of foramen magnum the outflow and inflow peak velocities were 2.47±0.98mm/s and 2.94±1.34mm/s respectively,and the quantities of outflow and inflow were 0.534±0.249ml/s and 0.530±0.250ml/s respectively,and the net flow volume of CSF was 0.004±0.002ml/s.At the level of C2 the outflow and inflow peak velocities were 4.72±1.86mm/s and 4.89±1.78mm/s respectively,and the volumes of outflow and inflow were 1.106±0.476ml/s and 1.101±0.476ml/s respectively,and the met flow volume of CSF was 0.005±0.003ml/s.Conclusion:MRI cine PC is a noninvasive method to study the CSF flow in physiological and pathological conditions for determining the pattern,direction,speed and quantity of the CSF flow.Therefore MRI cine PC is better than other invasive research modalities.
