摘要
目的 :对我院 1999年全年临床分离病原菌排在前 10位的菌类所做过药敏实验的多种抗菌药物敏感率进行分析 ,为临床经验用药提供更合理的选用抗菌药物的依据。方法 :肉汤稀释法测 MIC,通过 Sceptor数码分析系统完成鉴定和药敏实验。结果 :(1)临床分离菌株中的前 10位革兰氏阴性杆菌占 7个 ,革兰氏阳性球菌占 3个 ;(2 ) 7种革兰氏阴性杆菌对氨苄西林已基本耐药 ,敏感率均 <2 0 % ,加酶抑制剂的复方制剂也未能更好的拓宽抗菌谱 ;(3)革兰氏阴性杆菌对第二代头孢菌素敏感率并不优于第一代头孢菌素 ;(4 )铜绿假单胞菌对头孢他啶、头孢哌酮的敏感率分别为 80 %和 76 % ,大肠埃希氏菌、肺炎克雷伯氏菌对第三代头孢菌素的敏感率均在 6 0 %~ 93%之间 ,其他革兰氏阴性杆菌对第三代头孢菌素的敏感率约在 5 0 %以下 ,基本表现耐药 ;(5 )亚胺培南 /西司他丁尚保持较强的抗菌活性 ;(6 )多数革兰氏阴性杆菌对阿米卡星尚敏感 ;(7) 3种革兰氏阳性球菌对β-内酰胺类抗生素、喹诺酮类、红霉素等敏感率均 <5 0 % ;(8)出现了对万古霉素耐药的可疑的金葡球菌、表葡球菌菌株 ;(9)金葡球菌、表葡球菌、表葡球菌群对利福平的敏感率在 6 0 %~ 75 % ;(10 )嗜麦芽黄单胞菌、鲍氏不动杆菌耐药情况严重。结论 :抗生素敏感率下降 ,耐药?
In order to provide a current situation of the sensitivity rate of the 10 most common clinical isolates encountered in our hospital during the yearly of 1999 against antibacterial agents, an investigation was conducted. The broth serial dilution method was used to determine the MICs and the drug sensitivity test was judged by Sceptor analysis system. Results were as follows: (1) Among the 10 most common clinical isolates, 7 were Gram negative bacteria and 3 Gram positive cocci; (2) Ampicillin was basically resistant to all of the Gram negative bacteriama, the sensitive rate was lower than 20%; and even with the combined formulation containing the β lactamase inhibitor preparations were hardly alter the situation; (3) As for the second generation cephalosporins against Gram negative bacteria were not superior to those of the first generation ones; (4) The sensitive rates of the third generation cephalosporins, ceftazidime and cefaperazone against P.aeruginosa were 80% and 76%: to E.coli and K.pneumoniae were around 60% and 93% respectively, while the activity to other Gram negative bacteria the sensitive rates were lower than 50%; (5) lmipenem/cilastatin still posessed a strong inhibitory; (6) Amikacin was still sensitive against most of the Gram negative bacteria; (7) The sensitive rates of β lactams , quinolones and erythromycin against 3 species of Gram positive cocci were lower than 50%; (8) There was suspected the existence of vancomycin resistant S.aureus and S.epidermidis ; (9) The sensitive rate of rifampicin against S.aureus and S.epidermidis and their group species was around 60%~70%; (10) A serious resistance against Xanthomonas maltophilia and Acinetobacter baumannii was existed. Considering that the ever decreasing sensitivity of antibiotics to combat the bacterial/coccidal infections, the results of the survey study was worthwhile to remind health professionals of the rational usage of the antibiotic therapy
