机械性创伤患者血清sIL-2R的变化及其临床意义 被引量:2

摘要 目的 研究机械性创伤患者伤后血清可溶性白细胞介素 2受体 (sIL 2R)的动态变化及其与创伤评分的关系 ,探讨其机制及临床意义。方法 机械性创伤患者 5 0例 ,按创伤评分标准分成轻度、重度、极重度 3组。对其中 2 5例患者进行了 12d的动态观察 ,全部患者于伤后 2 4h内采血 ,动态观察组再于 3、5、7、9、12d各采血 1次。血清sIL 2R采用ELISA法测定。结果 创伤后 2 4h内各组sIL 2R明显高于正常值 (P <0 0 5 ) ,动态观察各组sIL 2R呈波动性先升后降的变化 ,轻度患者高峰值先于重度、极重度患者出现 ,sIL 2R的血清含量与创伤程度呈正相关 (r =0 3 0 6,P <0 0 5 )。结论 sIL 2R可作为判别创伤严重程度的灵敏指标 。 Objective To investigate the posttraumatic change of serum level of souble interleukin 2 receptor (sIL 2R) and its relationship with injury severity score(ISS) and to explore the mechanism and clincal significance. Methods According to the standard of ISS, 50 patients were divided into 3 groups:mild degree, severe degree and extremely severe degree groups. Of them, 25 patients were dynamically observed for 12 days. Blood samples in all patients were obtained within 24 hours, and on days 3,5,7,9 and 12 in dynamically observed patients. The sIL 2R was assayed by ELISA. Results The levels of sIL 2R were significantly higher in patients after trauma than in normal control within 24 hours ( P <0 05). The dynamical observation showed sIL 2R curve changes with early increase and later decrease. The climax of sIL 2R appeared earlier in mild group than in severe and extremely severe groups. The level of sIL 2R was correlated with the severity of trauma( r=0 306,P <0 05). Conclusion The level of sIL 2R is a sensitive index of the degree of trauma. The increase of sIL 2R shows the immunosuppression in traumatic patients.
