摘要
目的 研究乙型病毒性肝炎肝内ICAM 1和HLA A ,B ,C表达间的关系及其在乙型肝炎免疫发病机制中的作用。方法用免疫组化单种染色检测 11例正常人和 70例HBV感染者肝内ICAM 1、HLA A ,B ,C表达水平 ,并用免疫组化双重染色检测其肝内ICAM 1和HLA A ,B ,C复合表达。结果 乙型肝炎患者肝细胞ICAM 1表达水平与HLA A ,B ,C表达水平之间呈显著正相关 (P <0 .0 5 ) ;在炎症坏死区 ,表达ICAM 1的肝细胞多数也同时表达HLA A ,B ,C。结论 ICAM 1和HLA A ,B ,C共同参与介导乙型肝炎肝细胞的免疫损伤过程 ;ICAM 1/HLA A ,B ,C双阳性肝细胞可能是CTL发挥有效细胞毒作用的靶细胞。
Objective To study the relationship between intercellular adhesion molecule 1 (ICAM 1) and HLA A,B,C expression in the liver tissue of viral hepatitis B, and their role in the immunopathogenesis of viral hepatitis B. Methods The expression of ICAM 1 and HLA A,B,C in the livers of 11 healthy subjects and 70 HBV patients were detected by immunhistochemical single staining technique. The complex expression of ICAM 1 and HLA A, B, C was detected by immunhistochemical double staining technique. Results There were significant positive relation between the levels of ICAM 1 expression and the levels of HLA A,B,C expression in hepatocytes of the patients with viral hepatitis B ( P <0.05). In the areas of inflammation and necrosis, most hepatocytes expressed ICAM 1 and HLA A,B,C simultaneously. Conclusion ICAM 1 is involved in mediating immune damage of hepatocytes together with HLA A,B,C in viral hepatitis B. ICAM 1/HLA A,B,C double positive hepatocytes may be the target cells attacked by the cytotoxic T lymphocyte(CTL).
