摘要
目的 动态观察在拉米夫定治疗慢性乙型肝炎（CHB）中对患者外周血单核细胞(PBMC)培养上清中IFN-γ、IL-10的影响。 方法 选取接受拉米夫定治疗的CHB患者26例，分别在治疗前、治疗后1、3、6、9个月分离外周血单核细胞，在rHBcAg刺激下培养72 h， ELISA法检测上清中IFN-γ、IL-10含量。结果 未经治疗的CHB患者IL-10水平明显高于正常对照组(P<0.01)，IFN-γ生成量无显著差异(P>0.05)。拉米夫定治疗后各时间点，IFN-γ量较正常对照组和治疗前明显升高（P<0.05），而IL-10量减少，在治疗后第1、9月与正常对照组比较无明显差异（P>0.05）。完全应答组和部分应答组IFN-γ生成量均较治疗前高(P<0.05)，而IL-10的生成量仅在完全应答组降低明显(P<0.05)。 结论 对拉米夫定治疗发生完全应答的CHB患者，其Th1细胞应答增强，Th2细胞应答减弱。
Objective To study the effect of lamivudine on IFN-γ and IL-10 contents in the cell culture supernatant of peripheral blood mononuclear cells (PBMCs) from patients with B (CHB). Methods Before and at 1, 3, 6 and 9 months during lamivudine therapy, PBMCs from 26 patients with CHB receiving lamivudine treatment were isolated routinely and stimulated with rHBcAg, and IFN-γ and IL-10 contents in the cell culture supernatant measured by enzyme- linked immunoassay (ELISA). Results Before lamivudine treatment, the levels of IL-10 in the culture supernatants of PBMCs from the patients with CHB were significantly higher than that of controls (P<0.01), while the levels of IFN-γ did not (P>0.05). In the course of lamivudine treatment, the levels of IFN-γ increased at 1, 3, 6 and 9 months (P<0.05) in spite of the slight decrease of IL-10 (P>0.05). In patients fully or partially responding to lamivudine, the levels of IFN-γ were significantly increased as compared with those before therapy, and obvious decrease in IL-10 levels were observed only in patients that were fully responding to lamivudine. Conclusion Lamivudine treatment can promote a Th1 cytokine-mediated immune responses in patients with full response to lamivudine treatment.
