目的检测桥本氏甲状腺炎(HT)患者甲状腺组织及外周静脉血中Th17细胞和调节性T细胞(Treg)细胞相关细胞因子的表达水平,分析Th17/Treg免疫平衡在HT发病机制中的作用。方法采用逆转录,聚合酶链反应(RT—PCR)法,检测10例HT甲功正常组、10例HT甲低组和10例正常甲状腺组织(对照组)中IL-17A、Foxp3mRNA的表达水平;另采用ELISA法,检测上述三组血清中IL-17、IL-10的含量。结果与对照组相比,HT患者甲状腺组织内IL-17AmRNA的表达显著升高(t=0.5061,P〈0.05),且HT甲低组高于HT甲功正常组(t=0.2575,P〈0.05);HT患者甲状腺组织中Foxp3mRNA的表达降低(t=0.5298,P〈0.05),但其表达水平则与甲状腺功能无关。HT患者血清IL-17水平明显高于对照组(t=5.5840,P〈0.05),且HT甲低组高于HT甲功正常组(t=2.2760,P〈0.05);HT患者血清IL-10水平明显低于对照组(t=17.9170,P〈0.05),但HT甲低组与HT甲功正常组则差异无统计学意义(t=0.0410,P〉0.05)。结论HT患者甲状腺组织和外周血中Th17细胞应答增强,而Treg细胞免疫抑制下降;Th17/Treg免疫失衡可能参与HT的发生发展。
Objective To observe the expression of related cytokines of Th17 and CD4 ^+ CD25 ^+ regulatin7 T cells in the thyroid and peripheral blood of patients with Hashimoto' s thyroiditis, and to explore the role of Th17/Treg balance in the pathogenesis of HT. Methods Thyroid and peripheral blood were collected from 10 patients with HT who developed hypothyroidism, 10 patients with HT who were euthyroid and 10 control subjects without HT( control group). The expression of IL-17A and Foxp3 mRNA in thyroid were measured by RT- PCR. The levels of related cytokines IL-17and IL-10 in peripheral blood were assayed by ELISA. Results Compared with control group, in patients with HT, the expression of IL-17A mRNA in thyroid increased significantly ( t = 0.5061, P 〈 0.05 ), which increased markedly in hypothyroidism group than that in euthyroid group ( t = 0. 2575 ,P 〈 0.05 ) ; the expression of Foxp3 mRNA in thyroid decreased significantly in HT patients ( t = 0.5298,P 〈0.05) ,but it seemed to have nothing to do with thyroid function state. The serum levels of IL-17 in patients with HT increased significantly ( t = 5. 5840, P 〈 0.05 ) , which were higher in hypothyroidism group than that in euthyroid group( t = 2. 2760, P 〈 0.05 ) ; the serum levels of IL-10 decreased significantly in HT patinents (t = 17. 9170, P 〈 0.05 ), but there were no significant difference between hypothyroidism group and euthyroid group ( t = 0.0410,P 〉 0.05 ). Conclusion The increased response of Th17 ceils and diminished immunosuppression of Treg cells are presented both thyroid and peripheral blood in patients with HT, suggesting that the imbalance between Th17 and Treg may play an important role in the process of HT.
