摘要
目的 分析正常妇女不同月经时期乳腺超声影像特点。方法 5 2名 2 5~ 45岁月经正常、未发现乳腺疾患的妇女 ,记录近三个月经周期乳房胀疼出现及持续时间 ,于黄体期和晚卵泡期行超声检查 ,测量腺体层厚度、导管宽度、腺体结构及血流。结果 黄体期腺体层增厚 7% ,晚卵泡期导管增宽 11% ;出现腺体结构改变 2 6人 (5 0 % ) ,未生育者 13名中8人 (6 1.5 % )、生育未哺乳者 16名中 5人 (31.3% ) ,生育哺乳者 2 3名 13人 (5 6 .5 % )有腺体结构改变 ;卡方检验 ,无显著性差异 ;乳房胀疼 32人 (6 1.5 % ) ,皆于经前出现 ,平均持续 4天 ,其中 2 1人 (6 5 .6 % ) ,无乳房胀疼 5人 (2 5 % )有腺体结构改变 ,卡方检验 ,P <0 .0 1。结论 正常妇女乳腺腺体声像图表现随月经周期而变化 ;乳房胀疼与乳腺腺体结构改变相关 ;未生育者出现乳腺结构改变较生育者有增多的趋势。
Objective To determine the features of the breast ultrasound image during the different phase of menstrual cycle in women with normal menses. Methods Breast ultrasound was performed during luteal phase and late follicular phase in 52 women aged 25 to 45, with normal menstrual cycle and no distinct breast disease. Breast section thick, ductal width, breast structure and blood flow were measured. Initiating time and days of the cyclic breast pain were recorded. Results Breast section was thicker (increasing 7.3%) in luteal phase and breast ductal was wider (increasing 13%) in late follicular phase. Twenty six women (50%) had breast glandular structure change. Eight of 13 (61.5%) nulliparous women, five of 16 (31.3%) parous women without breast feeding and thirteen of 23 (56.5%) parous women who had breast feed had breast glandular structure change, there is no significant difference between three groups. Thirty two women (61.5%) had cyclic breast pain, Twenty one of them (65.5%) and five of 20 (25%) women without cyclic breast pain had breast glandular structure change, Chi square test, P <0.01. Conclusion Image of breast ultrasound changed following menstrual cycle in normal menses women. Breast glandular structure change is related to cyclic mastalgia. More nulliparous women had breast glandular structure change than parous women. [
