摘要
目的:分析乳腺浸润性导管癌的超声图像特征,探讨主要图像特征与发生腋淋巴结转移的关系。方法:收集资料完整的经病理证实的乳腺浸润性导管癌92例,分析研究肿瘤的高频声像图表现;并依据腋淋巴结病理结果分为腋淋巴结转移组与非转移组,统计两组患者年龄、肿瘤大小、纵横比、病灶边缘、周边状况、病灶内部回声、后方回声、血供丰富程度、血流阻力指数,探讨乳腺癌超声特征与腋淋巴结转移的关系。结果:乳腺浸润性导管癌术前超声诊断准确率为91.30%(84/92),其声像图特征主要表现为:乳腺内实质性低回声肿块、形态不规则、纵横比>1、边缘毛刺、周边高回声光带、微钙化、内部丰富血供及血流阻力>0.70。淋巴结转移组与非转移组在肿瘤大小、边缘毛刺、周边强光带、血供丰富程度方面有显著性差异(P<0.001)。结论:彩超在乳腺浸润性导管癌的诊断中具有特征性的表现,有较高的诊断准确性;综合分析淋巴结图像改变结合病灶典型特征对提示腋淋巴结转移具有诊断意义。
Objective To analyze the ultrasound characteristics of breast invasive ductal carcinoma,and to explore the relationship between main image characteristics and axillary lymph nodes metastasis.Methods The high frequency ultrasound image data of 92 patients with invasive ductal carcinoma confirmed by pathology were analyzed,and then the patients were divided into metastasis group and nonmetastasis group according to the status of axillary lymph nodes metastasis.The patients' age,tumor size,aspect ratio,edge of focus,surrounding condition,internal echo of lesion,posterior echo,level of blood supply,blood flow resistance index were compared between two groups.Results The accuracy of ultrasound for invasive ductal carcinoma was 91.30% postoperation,the main features of image were breast hypoechoic mass within the substantial,irregular form,aspect ratio>1,marginal spiculated sign,hyperechoic light band of surrounding,micro-calcification,rich blood flow and resistance index>0.70.There were significant difference in tumor size,marginal spiculated sign,hyperechoic light band of surrounding,rich blood flow between metastasis group and nonmetastasis group(P<0.01).Conclusion Color Doppler ultrasound may be helpful in diagnosing of invasive ductal carcinoma with its special imaging features,the characteristic of lymph nodes image combined the typical focus feature may play important roles in diagnosing axillary lymph nodes metastasis.
