摘要
目的 分析行眼内容剜除术的原因,探讨Ⅰ期羟基磷灰石义眼座植入术的手术效果.方法 回顾性分析对2000年1月至2008年7月在宜春市人民医院眼科,行眼内容物剜除术联合Ⅰ期羟基磷灰石义眼座植入术的连续性病例资料,分析其行眼内容物剜除术的原因,随诊观察义眼座植入和义眼片配戴后的效果.结果 61例患者中,48例（78.7％）为严重眼外伤患者,其中10例严重眼球破裂伤、9例外伤后眼内炎、29例严重眼外伤修补术后的并发症或后遗症（眼球萎缩、粘连性角膜白斑）;4例（6.5％）为感染性角膜溃疡;2例（3.3％）为绝对期青光眼;4例（6.5％）为眼部手术后并发症;3例（5％）为角膜葡萄肿.61例患者中55例（90％）为60岁以下的中青年患者.义眼座植入术后无感染等严重并发症,效果满意.术后佩戴亲水凝胶软性义眼片者舒适度更好.结论 作为基层医院,行眼内容物剜除义眼座植入术者主要为严重眼外伤患者,尤其是交通意外和生产意外导致的眼球破裂伤患者.眼内容剜除联合Ⅰ期羟基磷灰石义眼座植入术效果满意.
Objective To analyze the causes of evisceration,and to study the results and features of hydroxyapatite （HA） implantation.Methods Retrospectively study the patients underwent the evisceration and with the primary surgery of HA implantation.Results Sixty-one patients with the operation of evisceration and HA implantation were：48 （78.7%） cases of ocular trauma,such as severe eyeball rupture and endophthalmitis,4 （6.5%） of infective corneal ulcer,2 （3.3%） of absolute glaucoma,4 （6.5%） of side-effect of surgery and 3 （5%） of cornea staphyloma.Fifty-five cases of patients were younger than 60 years old.All the patients had good results without severe side effect.Conclusions Most of the patients underwent with evisceration and HA implantation is severe ocular trauma such as the eyeball rupture.The safety during the traffic and working should be emphasized to avoid and reduce the severe ocular trauma.The results of severe ocular trauma should be improved.
