摘要
目的：探讨达那唑联合糖皮质激素治疗难治性免疫性血小板减少性紫癜的疗效。方法39例患者分为激素治疗依赖组与激素治疗无效组。激素治疗依赖组：重新给予标准剂量激素，同时给予达那唑10 mg/（kg.d）（0.4~0.6 g/d），分2-3次口服；激素治疗无效组：激素继续治疗，同时给予达那唑10 mg/（kg.d），分2-3次口服；两组治疗观察时间均为6个月。结果激素治疗依赖组有效率为84%；激素治疗无效组有效率为55%，两组差异有显著统计学意义（P〈0.05）；两组总有效率为69%。结论达那唑联合糖皮质激素治疗难治性免疫性血小板减少性紫癜有效，且激素治疗依赖组明显优于激素治疗无效组。
Objective Our aim is to discuss the treatment efficacy of Danazol combined with glucocorticoid in refractory immune thrombocytopenic purpura. Methods Al 39 patients diagnosed as refractory immune thrombocytopenic purpura were divided into Glucocorticoid-independent group and Glucocorticoid-dependent group, which were defined as glucocorticoid resistance, and good treatment efficacy with glucocorticoid（dose-dependent）, respectively. Glucocorticoid-independent group：on the premise of primary glucocorticoid treatment, Danazol （10 mg/kg） was added. Glucocorticoid-dependent group：the standard dose of glucocorticoid was reused, and Danazol （10 mg/kg） was added. The observation time was six months. Results The effective rates were 84%and 55%in Glucocorticoid-dependent group and Glucocorticoid-independent group, respectively;the overal effective rates were 69%. The difference was significant （P〈0.05）. Conclusion Danazol combined with glucocorticoid had good treatment efficacy in refractory immune thrombocytopenic purpura, especial in the kind of glucocorticoid-dependent.
