摘要
【目的】探讨卵巢肿物剥除术中可吸收线缝合、双极电凝及超声刀三种方式止血对卵巢功能的影响。【方法】将150例行单侧良性卵巢肿瘤剥除术患者,按不同止血方式分为超声刀组、双极电凝组、可吸收线缝合组,每组50例,分别于术前,术后1、3、6个月的月经周期第2-3天监测性激素(FSH、LH、E2)及窦卵泡数(AFC),随访术后1-6个月期间月经变化。【结果】3组患者术后均出现E2下降和FSH、LH增高及窦卵泡数减少,与术前比较均有显著性差异(P〈0.05);缝合组术后3个月恢复术前水平,超声刀组术后6个月恢复,而双极组术后激素水平及窦卵泡数虽逐渐恢复,但相对缓慢。月经改变主要表现为周期延长、经量减少,偶有经期延长及轻微潮热出汗,情绪烦躁的表现,但术后逐渐缓解。【结论】卵巢肿物剥除术中可吸收线缝合对卵巢功能影响最小而恢复最快,双极电凝止血对卵巢功能影响最大、恢复最慢。
【Objective】To investigate the influence on the ovarian function using three diverse hemostasis in ovarian cystectomy. 【Methods】We conducted 150 women undergoing ovarian cystectomy in the study. They were divided randomly into three groups of 50 cases each, according to their hemostasis: the ultrasonic scalpel group, the bipolar coagulation group and the absorbable suture group. Then, venous blood samples at the second and third day of menstrual cycle and the amounts of follicles were detected 1 month, 3 months, and 6 months before and after the operation, separately. Besides, all cases were followed up to obtain the menstrual condition in thefollowing 6 months.【Results】After the operation, the amount of E2(the estradiol) and AFC(antral follicle count) have decreased and the amount of FSH(follicle stimulating hormone) and LH(luteinizing hormone) have increased in the three groups of patients. The result after the operation has significant difference(P 0.05)from the preoperative one. The ovarian function ofabsorbable suture group recovered to the preoperative level 3 months after the operation and the ultrasonic scalpel group recovered 6 months after that.While the three kinds of sex hormones and AFC of the bipolar coagulation group recovered relatively slowly. The ultrasonic scalpel group recovered 6 months after that.As for the alteration of menstruation, the main aspects are the delay of menstrual cycle,the decrease of menstrual blood volume and the occasional hot flash and irritable depression.【Conclusion】For the ovarian function in ovarian cystectomy, the absorbable suture had the least influence and the fastest postoperative recovery,while the bipolar coagulation has the greatest influence and the slowest postoperative recovery.
