摘要
目的探讨腹腔镜治疗黄体破裂出血手术中不同止血方法对卵巢功能的影响。方法选取186例因黄体破裂出血接受腹腔镜治疗的患者,根据术中不同止血方法,分为单极电凝组(A组)、双极电凝组(B组)以及缝合止血组(C组)。对比3组患者术前及术后E2、FSH和LH水平。结果 (1)3组患者术后1个月时与术前比较E2水平下降,而FSH和LH水平增高(P<0.05);术后3个月时与术前比较A组和B组E2、FSH和LH水平有所恢复(P<0.05),而C组E2、FSH和LH水平基本恢复术前水平(P>0.05)。(2)术后3个月内,A组出现月经异常者47例(71.21%),B组25例(37.88%),C组14例(21.21%)。结论电凝止血对卵巢功能的影响大于缝合止血,而且单极电凝比双极电凝对卵巢功能影响更大。
objective To study the influence of different hemostasis to ovarian function in laparoscopic surgery treatment corpus luteum rupture. Methods According to different hemostasis, 186 patients were divided into unipolar electricity group(group A), bipolar electricity group(group B)and the suture bleeding group(group C). The preoperative and postopera- tive E2, FSH and LH level were contrasted. Results (1)In 3 groups, compared with the preoperative, E2 levels dropped, and FSH and LH levels increased after one month (P 〈 0.05); After 3 months E2, FSH and LH level of group A and B re- sumed (P 〈 O.05),and E2, FSH and LH levels of group C recoveried preoperative levels (P 〉 0.05). (2)In postoperative 3 months, 47 patients with abnormal menstrual in group A (71.21%), 25 patients (37.88%) in group B, 14 cases (21.21%) in group C. Conclusion The influence of eleetrocautery for ovarian function is greater than suture bleeding, and the influence of unipolar electricity for ovarian function is greater than bipolar electricity.
