脑苷肌肽在新生儿缺血缺氧性脑病中的应用效果研究 被引量:1

Clinical study of cattle encephalon glycoside and ignotin injection on neonatal hypoxic-ischemic encephalopathy

摘要 目的观察脑苷肌肽在新生儿缺氧缺血性脑病(hypoxie-ischemic encephalopathy,HIE)中的应用效果。方法选择2010年1月—2013年11月住院治疗的HIE患儿68例,根据随机数字表法分为对照组和观察组各34例,两组均给予支持疗法、维持脑和全身良好的血流灌注、维持血糖水平、控制惊厥,维持水、电解质平衡。对照组在以上治疗基础上加脑活素注射液2.5 ml加入5%葡萄糖注射液30 ml中静脉滴注,1次/d。观察组加用脑苷肌肽注射液2ml加入5%葡萄糖注射液30 ml中静脉滴注,1次/d。两组均连续治疗10 d。对比两组疗效及治疗前后血浆高敏C反应蛋(high-sensitive Creactive protein,hs-CRP)、白细胞介素18(interleukin 6,IL-18)和肿瘤坏死因子α(tumor necrosis factorα,TNF-α)水平。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果总有效率对照组61.76%,观察组91.18%,两组比较差异有统计学意义(χ2=8.173,P<0.05)。治疗后hs-CR、IL-18、TNF-α水平均较治疗前降低,但观察组[(2.54±0.73)mg/L、(132.56±46.57)、(45.61±12.08)pg/ml]显著低于对照组[(4.56±1.30)mg/L、(266.50±63.58)、(69.36±15.48)pg/ml],比较差异均有统计学意义(t=7.900、9.910、7.053,均P<0.05)。结论脑苷肌肽治疗新生儿HIE可显著抑制炎症状态,改善症状,提高临床疗效。 Objective To observe the clinical effects of cattle encephalon glycoside and ignotin injection in treating hypoxic - ischemic encephalopathy(HIE) in neonates. Methods 68 cases of HIE treated from January 2010 to November 2013 were divided into control group and observation group with random number table methods, 34 cases for each group. Both groups were given supportive therapy to maintain normal cerebral and systemic perfusion, blood sugar levels, and to control convulsion, maintain water and electrolyte balance. In control group,2.5 ml of cerebrolysin was given in mixture with 30 ml of 5% glucose solution once daily; and in observation group, 2 ml of cattle encephalon glycoside and ignotin injection was given in mixture with 30 ml of 5% glucose solution. After a 10 -day treatment, both groups were treated for ten consecutive days and compared for clinical efficacy and the level of hs - CRP,IL - 18 and TNF - a. T - test was used for measurement data,andx2 test for count data,the result of P 〈 0.05 indicated that the difference was statistically significant. Results The total effective rate was 61.76% in control group and 91.18% in observation group,which showed statistically signifi- cant difference (x2 = 8. 173 ,P 〈 0.05 ). Levels of hs - CR, IL - 18 and TNF - ct decreased in both groups after treatment, and they were significantly lower in observation group than in control group [ ( 2.54 ± 0.73 ) mg/L, ( 132.56 ± 46.57 ) pg/ ml, (45.61 ± 12.08) pg/ml versus (4.56± 1.30) mg/L, (266.50 ± 63.58 ), (69.36 ± 15.48) pg/ml ], wbieh showed sta- tistically significant difference ( t = 7. 900, 9. 910, 7. 053, P 〈 0.05) . Conclusions Cattle encephalon glycoside and ig- notin injection used in neonatal HIE can significantly inhibit inflammation, improve symptoms and clinical efficacy.
