摘要
目的比较不同手术方式治疗腰椎结核的临床疗效。方法回顾分析2007-01-2012-12收治的162例腰椎结核患者,A组(后路手术组)72例,行一期后路病灶清除、植骨融合内固定术;B组(前后路联合手术组)57例,行一期前路病灶清除、植骨融合后路内固定术,前路手术,C组(前路手术组)33例,行一期前路病灶清除、植骨融合内固定术,观察比较分析三组患者的手术时间、术中出血量、住院天数及手术前后的Frankel分级、Cobb角、血沉变化情况。结果所有病例切口均I期愈合,无严重并发症发生,手术时间、住院天数B/A,C/A比较有显著性差异P<0.01,B/C比较无明显差异P>0.05,另外三组的Frankel分级、Cobb角及ESR值变化程度比较无明显差异P>0.05,结论一期后路病灶清除、植骨融合内固定术治疗腰椎结核与前后路联合及前路术式均可获得治疗效果,但后路术式手术时间短、出血少、住院时间短。
Objective To compare the clinical effect of different surgical methods in the treatment of tu- berculosis of lumbar spine. Methods The clinical data of 162 cases with lumbar tuberculosis who admitted in our hospital from January 2007 to December 2012, were retrospectively studied. 72 cases underwent posterior focus clearance,bone graft fusion and internal fixation at one-stage were selected as group A. 57 cases experi- enced anterior debridement and bone graft plus posterior instrumentation were selected as group B. 33 cases re- ceived anterior focus clearance,bone graft fusion and internal fixation at one-stage were selected as group C. The operation time,intraoperative blood loss,hospitalization,Frankel grade and Cobb＇s angle, ESR before and af- ter surgery were compared among the three groups. Results The wound primary healing achieved in all pa- tients, no severe complications were found in three groups. The operation time and the average blood loss showed significant differences between A,B groups and A,C groups （P〈0.01）,but no significant differences be- tween B,C groups （P〉0.05）. No significant differences were found in Frankel grade,Cobhs angle and ESR a- mong three groups （P〉0.05）. Conclusion Operation of lumbar tuberculosis by posterior focus clearance, bone graft fusion and internal fixation at one-stage, the surgery combined anterior and posterior and anterior surgery can all get treatment effect,but posterior surgery has the advantages of less operation time,less blood loss and shorter hospitalization. So it is a feasible, effective surgical treatment.
