摘要
【目的】观察益发口服液合乌发生发酊对肝肾不足型斑秃患者免疫功能的影响。【方法】将100例肝肾不足型斑秃患者随机分为治疗组和对照组各50例,治疗组给予益发口服液口服合乌发生发酊外用治疗,对照组给予常规西药治疗(胱氨酸、维生素B6、谷维素内服和盐酸氮芥外用),观察2组临床疗效,采用双抗体夹心酶联免疫吸附反应(ELISA)法检测2组肝肾不足型斑秃患者治疗前后外周血T淋巴细胞亚群(CD4+、CD8+)及细胞因子[干扰素-γ(IFN-γ)、白细胞介素-12(IL-12)、白细胞介素-10(IL-10)]水平,并与30例正常对照组比较。【结果】(1)经过2个疗程的治疗,治疗组总有效率为84.0%,对照组为62.0%,治疗组疗效优于对照组(P﹤0.05)。(2)治疗前,2组患者的血清IFN-γ、IL-12和T淋巴细胞亚群CD4+、CD8+水平均高于正常组,IL-10水平均低于正常组(P﹤0.05)。治疗后,治疗组血清IL-12、IFN-γ和T淋巴细胞亚群CD4+、CD8+水平降低,IL-10水平升高,与治疗前比较差异有统计学意义(P﹤0.05),且基本恢复至正常组水平(P>0.05);对照组血清IL-12、IFN-γ和T淋巴细胞亚群CD4+、CD8+水平有降低趋势,但差异均无统计学意义(P>0.05);2组治疗后比较,治疗组各项指标的改善作用均优于对照组,差异均有统计学意义(P﹤0.05)。【结论】斑秃患者存在免疫功能紊乱,益发口服液合乌发生发酊对肝肾不足型斑秃患者的免疫功能有一定的调整作用,这可能是其治疗斑秃的作用机制之一。
Objective To observe the effect of Yifa Oral Liquid and Wufa Tincture on the immune function of alopecia areata patients with liver and kidney deficiency syndrome. Methods One hundred alopecia areata patients with liver and kidney deficiency syndrome were evenly randomized into treatment group and control group. The treatment group was given Yifa Oral Liquid orally and Wufa Tincture externally. The control group was given oral use of cystine, Vitamin B6 and oryzanol, and external application of chlormethine hydrochloride. After treatment, thetherapeutic effect was evaluated in both groups. The peripheral levels of T lymphocyte subsets （CD4 ＋ and CD8＋） and cytokines of interferon gamma （IFN-y）, interleukin 12 （IL-12） and interleukin 10 （IL-10） were measured by double-antibody sandwich enzyme-linked immunosorbent assay （ELISA） method before and after treatment, and were compared with those of 30 normal subjects. Results （1） After treatment for 2 courses, the total effective rate was 84.0% in the treatment group and was 62.0% in the control group, the difference being significant （P〈 0.05）. （2） Serum IFN-y, IL-12, CD4＋ T, and CD8＋ T levels were higher while IL-10 in both groups was lower than those in the normal control group before treatment （P〈 0.05） . After treatment, IL-12, IFN-y/, CD4＋ T and CD8＋ T levels were decreased but IL-10 was increased in the treatment group （P 〈 0.05 compared with those before treatment）, and the levels almost arrived to the normal level （P〉 0.05） . In the control group, IL-12, IFN-＇y, CD4＋ T and CD8＋ T levels showed decreasing trend, but the differences were insignificant （P〉0.05） . After treatment, the improvement of the observation indexes in the treatment group was superior to that in the control group, the difference being significant （P〈0.05） . Conclusion Immune function disorder exists in alopecia areata patients. Yifa Oral Liquid and Wufa Tincture have regulatory effect on the immune function of alopecia areata patients with liver and kidney deficiency syndrome, which may be one of their therapeutic mechanisms in the treatment of alopecia areata.
