The expression and clinical significance of matrix metalloproteinase-8 in the acute Stanford A aortic dissection

摘要 目的探讨基质金属蛋白酶-8(MMP-8)在急性Stanford A型主动脉夹层中的表达及临床意义。方法 30例急性Stanford A型主动脉夹层患者为实验组,30例同期接受单纯主动脉瓣置换的患者为对照组,采用ELISA法、Western blot法、免疫组化法等检测MMP-8在实验组和对照组血浆及升主动脉壁中的表达。结果主动脉夹层患者血浆中MMP-8的表达[(106.51±28.27)ng/ml]明显高于对照组[(25.65±12.95)ng/ml],差异有统计学意义(P<0.05);实验组升主动脉中层病变显著,炎性细胞浸润夹层边缘。弹性纤维与胶原纤维层变性、排列紊乱、破碎、崩解。其主动脉壁中层MMP-8大量表达。而对照组主动脉壁结构完整,弹性纤维与胶原纤维排列紧密有序,无中层退行性变。MMP-8在其主动脉中层仅少量表达或不表达。结论 MMP-8在病变的升主动脉中层内高表达,在正常的弹性纤维层低表达,提示其在主动脉疾病的发生发展中可能发挥作用。 Objective To investigate the expression and clinical significance of matrix metalloproteinase-8 ( MMP-8 ) in the acute Stanford A aortic dissection. Methods Blood samples were collected from 30 acute Stanford A aor-tic dissection patients and 30 aortic valve replacement patients. ELISA was used to measure the contents of MMP-8 in the plasma. The tissue expressions of MMP-8 in the surgically obtained samples were detected by Western blot and immunohistochemistry. Results The acute Stanford A aortic dissection patients showed a significantly higher level of MMP-8 in the plasma [ ( 106.51 ±28.27 ) ng/ml ] than the aortic valve replacement patients [ ( 25.65 ±12.95)ng/ml](P〈0.05). Significant lesions of aortic intima, inflammatory cell infiltration laminated edge, elas-tic fibers and collagen fiber fragmentation, disintegration were observed in the experimental group, but not in the control group. The MMP-8 was significant expression in the experimental group than in the control group. Conclu-sion The MMP-8 is highly expressed in the middle of aortic lesions but lowly expressed in the normal fibrous tissue layer suggesting that it may play a role in development of the aortic disease.
