摘要
目的通过观察比较多囊卵巢综合征(PCOS)患者治疗前后的内分泌代谢指标的变化,探讨达英-35与二甲双胍联合用药对PCOS患者内分泌系统的影响及其治疗效果。方法对妇科门诊90例PCOS患者采用达英-35与二甲双胍联合治疗,比较治疗前、后患者临床症状与月经恢复情况,血清卵泡刺激素(FSH)、雌二醇(E2)、黄体生成素(LH)、孕酮(PRGE)、垂体泌乳素(PRL)、睾酮(T)、空腹胰岛素(FIN)的变化情况。结果联合用药治疗后有65例患者月经恢复规则,63例患者的多毛痤疮症状明显改善;患者的血清LH降至(8.54±1.88)mU/mL,T降至(38.81±6.41)ng/dL,FIN降至(15.44±3.25)mU/L,均明显低于治疗前,差异有高度统计学意义(P<0.01);而血清E2、FSH、PRGE、PRL的检测值治疗后均与治疗前比较差异无统计学意义(P>0.05)。结论达英-35联合二甲双胍用药能有效降低PCOS患者LH、T及FIN水平,减轻胰岛素抵抗和高雄激素症状,显著改善患者内分泌环境,是治疗PCOS较为理想的有效方法。
Objective To observe and compare the polycystic ovary syndrome （PCOS） changes of endocrine metabolism in patients before and after treatment,and to discuss the influence and clinical effect of Diane-35 and Metformin combination therapy in the endocrine system of patients with PCOS.Methods 90 cases of PCOS patients with gynecological clinic were treated with Diane-35 and Mefformin,the clinical symptoms and menstrnal recovery before and after treatment of serum follicle stimulating hormone （FSH）,estradiol （E2）,luteinizing hormone （LH）,progesterone （PRGE）,prolactin （PRL）,testosterone （T） and fasting insulin （FIN） were compared.Results After the combination therapy,the menstrnation of 65 patients became rule,the hirsutism acne symptoms of 63 patients improved significantly; the serum LH was decreased to （8.54±1.88） mU/mL,T was decreased to （38.81±6.41） ng/dL,FIN was decreased to （15.44±3.25） mU/L,which were significantly lower than those before therapy,the differences were statistically significant （P ＜ 0.01）; the estimated value of serum E2,FSH,PRGE,PRL value after treatment were compared with those before treatment had no significant differences （P ＞ 0.05）.Conclusion Diane-35 combined with Mefformin treatment can reduce the PCOS of patients with LH,T and FIN levels,insulin resistance and Kaohsiung hormone symptoms,significantly improve the endocrine environment,which is an ideal effective method to treat PCOS.
