摘要
目的观察非动脉炎性前部缺血性视神经病变(NAION)的危险因素。方法对22例(22只眼)NAION患者的24h血压、心率变化进行监测,以及对双眼视盘相干光断层扫描(OCT)、视野改变等进行随访观察。结果24h血压、心率监测0:00—7:00点血压,尤其舒张压均低于白天水平,O:00~4:00心率低于白天水平。双眼视盘患眼中Ⅰ级杯11只眼,Ⅱ级杯9只眼,Ⅲ级杯2只眼。对侧未发病眼Ⅱ级杯6只眼,Ⅲ级杯8只眼,Ⅳ级杯4只眼。视野检查半侧缺损8只眼,上半侧缺损2只眼,鼻侧缺损2只眼,颞侧缺损1只眼,弓形缺损1只眼;且存在视敏度的降低。结论小视杯、视杯缺如或狭窄者应作为NAION的监测对象;夜间低血压、慢心率可导致视神经供血障碍,引发NAION的发生。
Objective In this study we investigated the risk factors of ischemic optic neuropathy. Methods We continuous monitored blood pressure and heart rate in 22 patients who suffered from iscbemic optic neuropathy. Binocular disk OCT and visual field changes of these patients were followed. Results Blood pressure, particularly diastolic pres- sure, was lower in NAION patients during 0:00-7:00 AM. Heart was also lower during 0:00-4:00 AM. Fundus examina- tion revealed grade I cup in 11 eyes, grade II cup in 9 eyes, and grade III cup in 2 eyes. Increased cup diameters were also found in contralateral eyes in NAION patients ( 6 with grade II cup, 8 with grade III cup, and 4 with grade IV cup ). Vari- ous degrees of visual field defects were identified in NAION. Specifically, hemi-defect was found in 8 eyes, upper defect in 2 eyes, nasal defect in 2 eyes, areuate defect in 1 eye, and segmental defect in 1 eye. Visual acuity was also decreased. Conclusion Patients with small optic cup,optic cup stenosis or without optic cup should be considered as the monitoring objects. Nocturnal hypotenson and slow heart rate can result in impaired blood supply of the optic nerve, will thus cause NAION attack.
