关节镜辅助小切口内固定治疗胫骨平台骨折39例 被引量:25

Clinical study of 39 tibial plateau fractures treated with mini-invasive method assisted by arthroscopy

摘要 目的探讨关节镜辅助下小切口内固定手术治疗胫骨平台骨折的可行性及疗效。方法对39例胫骨平台骨折在关节镜辅助下行小切口内固定手术治疗。术中根据骨折类型分别采用松质骨螺钉固定、经皮微创钢板内固定技术(MIPPO)置人LCP钢板。术后积极康复训练,定期复查患膝关节X线片。结果39例均获随访6~18个月,平均9个月。术后第1天膝关节屈曲达90°。39例骨折均获得临床愈合,未发生并发症。末次随访时膝关节功能Lysholm评分平均(94.04±4.71)分,较术前平均(50.81±9.80)分显著提高,差异有统计学意义(P〈0.05)。结论关节镜辅助下小切口内固定治疗胫骨平台骨折能提供关节内良好视野,有助于骨折更好地复位和固定。该方法具有创伤小、功能恢复良好等优点,值得推广应用。 Objective To investigate the feasibility and efficacy of the tibial plateau fracture treated with mini-invasive method assisted by arthroseopy. Methods A total of 39 eases of tibial plateau fracture were treated with mini-invasive method assisted by arthroscopy, aeeording to the types of fractures, using cancellous bone screw fixation, LCP plate implanted through MIPPO technology. Flexion-extension exercise and muscle contraction exercise of knee joint were performed on the first day after operation. Rehabilitation exercises were actively performed in the middle and latest stage after operation. In the 2nd month and the 6th month, the X-ray examination of knee was performed. Results All of 39 patients were followed up for 6-18 months after operation, with an average of 9 months. In the first day after operation, the flexion degree of knees could reach to 90°. Thirty nine patients all got clinical healing without complieations. Funetion of those knees was evaluated with Lysholm scales. There were signifieantly statistieal differences between pre-operation and post-operation(P 〈0.05). Conclusion The minimally invasive treatment assisted by arthroseopy for the fi'acture of tibial plateau can provide good inner-vision of the knee joint. It is helpful for better reduction and fixation,with the advantages of litde trauma, less ratio of infection, less interruption of the knee joint and faster rehabilitation. It is an effective method to treat tibial plateau fractures and worthy of application and dissemination.
