乳腺浸润性导管癌中VDR及OPN的表达与钙化的关系及临床意义 被引量:2

The relationship and clinical significance of osteopontin protein and vitamin D receptor expressions and calcification in infiltrating ductal breast cancers

摘要 目的 通过检测维生素D受体(VDR)及骨桥蛋白(OPN)在乳腺纤维瘤及浸润性导管癌组织中的蛋白表达情况,探讨VDR、OPN与乳腺钙化征及乳腺癌发生与进程之间的关系.方法 应用免疫组织化学链霉菌抗生物素蛋白-过氧化物酶连结法(S-P法)对60例钙化与非钙化型乳腺浸润性导管癌组织、20例乳腺纤维瘤组织分别进行OPN、VDR蛋白表达检测.结果 乳腺浸润性导管癌中VDR的表达显著高于良性肿瘤(Z=-2.724,P=0.006),患者不同年龄、绝经与否、肿瘤大小、组织学分级以及不同雌激素受体(ER)、孕激素受体(PR)、人类表皮生长因子受体2(HER-2)表达程度之间的VDR蛋白表达差异无统计学意义(P>0.05),而腋窝淋巴结转移与否、临床分期、钙化与否及不同OPN表达程度之间差异有统计学意义(P<0.05),并呈正相关(r=0.642,0.467,0.283,0.582,P<0.01,P<0.05);乳腺浸润性导管癌中OPN的表达显著高于良性肿瘤(Z=-2.627,P=0.009),患者不同年龄、是否绝经、临床分期、组织学分级以及不同ER、PR、HER-2的表达程度之间的OPN蛋白表达差异无统计学意义(P>0.05),而腋窝淋巴结转移与否、肿瘤大小及钙化与否之间差异有统计学意义(P<0.05),与腋窝淋巴结转移及钙化呈正相关(r=0.792,P=0.000;r=0.314,P=0.015),与肿瘤大小无关(P>0.05).结论 VDR、OPN在乳腺浸润性导管癌中高表达,提示可能参与了乳腺浸润性导管癌的发生发展及钙化的过程. Objective To investigate the expressions of osteopontin protein (OPN) and vitamin D receptor(VDR) in breast fi- bromas and infiltrating ductal breast cancers, and analyze its relationship with calcification and development of breast cancers. Methods Immunohistochemical streptavidin-perosidase (S-P) method was used to examine the expressions of OPN and VDR in 60 cases of infil- trating ductal breast cancers with and without calcification, and 20 cases of breast fibroadenomas. Results In infihrat!ng ductal breast cancers, VDR expressions were significantly higher than breast fibroadenomas ( Z = -2. 724, P = 0. 006). No obviously statistical significance was found between VDR protein expressions and the parameters (patient age, whether menopause, tumor size, histological grade, estrogen receptor (ER), progesterone receptor (PR), and human epidermalgrowth factor recptor-2 (HER-2) ), but significant differences existed among the axillary lymph node metastasis, tumor node metastasis (TNM) stage, calcification, and OPN expressions. The VDR protein expression was positively correlated with axillary lymph node metastases( r = 0. 642, 0. 467,0. 283, 0. 582, P 〈 0. 01, P 〈 0. 05), TNM stage, and cacification. OPN expressions in infiltrating ductal breast cancers were significantly higher than breast fibro- adenomas ( Z = -2. 627, P = 0. 009). No obviously statistical significance was found between OPN protein expression and the parameters (patient age, whether menopause, TNM stage, histological grade, ER, PR, and HER-2), but significant differences existed in the axillary lymph node metastasis, tumor size., and calcification. The OPN protein expression was positively correlated with axillary lymph node metastases and calcification ( r =0. 792, P =0. 000; r =0. 314, P =0. 015), but it had no significant correlation with tumor size ( P 〉 0. 05). Conclusions The high expressions of OPN and VDR in infiltrating ductal breast cancer suggest that OPN and VDR may be involved in the processes of occurrence and development and the calcification of infiltrating ductal breast cancers.
