摘要
目的探讨钛合金人工听小骨与自体组织在开放式鼓室成形听力重建中的疗效。方法将2008年8月至2012年8月入住解放军第十五中心医院的100例接受开放式鼓室成形术的患者按照抽签法随机分为对照组和观察组,各为50例。对照组给予自体骨组织治疗,观察组给予钛合金人工听小骨治疗,听力统计按照0.5,1.0,2.0及4.0 kHz的评价气骨导差值进行计算、分析。结果对照组手术前后气导值分别为(44.21±11.30)dB、(42.31±10.78)dB,两者比较差异无统计学意义(P>0.05);观察组手术前后分别为(46.77±12.98)dB、(28.65±5.68)dB,两者差异有统计学意义(P<0.01),且观察组术后气导值较对照组治疗后的差异有统计学意义(P<0.01)。对照组手术前后气骨导差值分别为(38.77±10.92)dB、(35.09±9.66)dB,两者比较差异无统计学意义(P>0.05),观察组手术前后分别为(41.39±13.20)dB、(14.82±3.20)dB,两者比较差异有统计学意义,且观察组术后气导值和气骨导差值较对照组术后显著降低(P<0.01)。结论钛合金人工听小骨在开放式鼓室成形听力重建中的疗效显著优于自体骨组织治疗,应在临床上加以推广并应用。
Objective To explore the titanium artificial osslcles and autologous tissue in reconstruction of open tympanoplasty hearing. Methods A total of 100 cases of patients who underwent open tympanoplasty surgery in PLA 15th Central Hospital were randomly divided into a control group of 50 cases and an observation of 50 cases group, with the balloting method during Aug. 2008-Aug. 2012. The control group received autolo- gous bone tissue treatment, and the observation group was given the titanium artificial ossicular treatment. And hearing statistics was calculated and analyzed according to the air-bone difference at 0.5,1.0,2.0 and 4.0 kHz. Results Air conduction values of the control group were respectively(44.21 ± 11.30) dB, (42.31± 10.78 ) dB before and after surgery, and there was no statistical difference (P 〉 0.05 ). The corresponding data of the observation group were respectively (46.77 ±12.98 ) dB, (28.65 ± 5.68 ) dB before and after sur- gery, and there was a significantly statistical difference (P 〈 0.01 ), and there was a significantly statistical difference(P 〈0.01 ) of postoperative air conduction value between the observation group and the control group after treatment. The air-bone difference of the control group were respectively (38.77 ±10.92 ) dB, ( 35.09 ± 9.66 ) dB before and after surgery, and there was no statistical difference ( P 〉 0.05 ), and of the observation group were respectively(41.39 ± 13.20) dB, ( 14.82 ±3.20) dB before and after surgery,which was a statistically significant difference, and there was a significantly statistical differencebetween the observa- tion group and the control group after treatment ( P 〈 0.01 ). Conclusion The titanium artificial ossiclcs have a significant better efficacy than autologous bone tissue in open tympanoplasty hearing reconstruction, thus should be promoted and applied in clinical practice.
