摘要
目的:观察加味四逆汤治疗结直肠癌患者化疗后血小板减少的临床疗效,并探讨其升血小板的作用机制。方法:将60例结直肠癌化疗后血小板减少的患者随机分为治疗组和对照组,每组各30例,治疗组予加味四逆汤治疗,对照组予重组人白介素-11(rhIL-11)治疗,比较2组患者治疗前后PLT、WBC、NF、HB及血清骨髓造血相关调控因子(IL-3、IL-6、IL-10、IL-11、TPO)的测定结果。结果:治疗组治疗后血小板水平明显高于治疗前,与对照组相比,血小板上升趋势无明显差异;治疗组WBC、NF、HB治疗前后无明显差异;治疗组治疗后IL-6浓度显著增加;治疗组不良反应明显少于对照组,耐受性显著提高。结论:1.加味四逆汤可显著提升血小板水平,且疗效不劣于IL-11,且安全性较IL-11高;2.加味四逆汤可以通过调控IL-6等的水平来发挥升血小板的作用。
Objective: To observe the clinical curative effect of Jia-wei-si-ni decoction treated thrombocytopenia after chemotherapy in colorectal cancer,and investigating its mechanism to enhance the platelet level. Methods: 60 patients met the inclusion criteria were randomly divided into 2 groups: The treatment group with 30 cases( treated with Jia-wei-si-ni decoction) and the control group with 30 cases( treated with recombinant human enterleukin-11). 2 groups were recorded the levels of platelet( PLT),white blood cell count( WBC),neutrophil count( NF),hemoglobin( HB),bone marrow hematopoietic regulatory factors of serum( IL-3, IL-6,IL-10,IL-11,TPO) before and after treatment. Results: By vertical comparison,the levels of platelets in 2 groups were increased in 7 days after treatment,which showed statistical difference compared with before treatment. By parallel comparison,the levels of platelets in the treatment group after treatment showed no significant compared with that of control group. The levels of WBC, NE,HB in the treatment group after showed no statistical difference compared with before treatment. Compared the bone marrow hematopoietic regulatory factors of serum( IL-3,IL-6,IL-10,IL-11,TPO) before and after treatment,the levels of IL-6 in the treatment group were increased,which showed statistical difference compared with before treatment. Conclusion:( 1) Jia-wei-si-ni decoc-tion can raise the level of platelet,which shows no statistical difference compared with the control group and it was much safer than the control group.( 2) Jia-wei-si-ni decoction plays the role of increasing platelet by regulating the bone marrow hematopoietic regulatory factors of serum such as IL-6.
