髋关节翻修治疗骨缺损应用组配式假体与涂层长柄假体临床疗效的Meta分析 被引量:6

Meta-analysis of clinical effect in revision hip arthroplasty to treat bone loss with modular stem and coated long stem

摘要 [目的]通过Meta分析评价两种非骨水泥假体:生物型组配式假体与涂层长柄假体,在髋关节翻修术治疗骨缺损时临床疗效的差异。[方法]文献检索PubMed、Ovid、 CochraneLibrary、 ScienceDirect、 SpringerLink数据库,根据纳入和排除标准筛选文献,收集组配式假体与涂层长柄假体在髋关节翻修术中应用的高质量随机对照研究,提取相关数据,采用Review Manager5.0软件进行Meta分析。[结果]共纳入8篇随机对照研究(Jadad评分3~5分)。Meta分析结果显示,组配式假体与涂层长柄假体在髋关节翻修术后并发症的发生率无统计学差异RR=1.12,95%CI(0.96,1.32),P=0.15;组配式假体在髋关节翻修术后假体塌陷的发生率低于涂层长柄假体RR=0.28,95%CI(0.08,0.95),P=0.04;术后疼痛的发生率也低于涂层长柄假体RR=0.45,95%CI(0.22,0.94),P=0.03;两种假体在髋关节翻修术后假体无菌性松动的发生率无统计学差异RR=1.39,95%CI(0.21,9.39),P=0.74。[结论]髋关节翻修术应用组配式假体相对于涂层长柄假体可以减少术后疼痛和假体塌陷的发生率。 [Objective] To evaluate the overall clinical effect of revision hip arthroplasty to treat bone loss with modular stem and coated long stem in uncemented. [ Method ] The literatures were searched from PubMed, Ovid, CochraneLibrary, Sci- enceDirect and SpringerLink. The data were extracted under the selection criteris and only the high quality randomized controlled trails were enrolled. The data were analyzed by using the Review Manager 5.0 to compare the difference of bone loss between modular stem and coated stem. [ Result] Eight randomized controlled trails were included as high quality. The risk between mod-ular stem and coated stem in postoperative complications ( RR = 1.12,95% CI ( 0.96,1.32 ), P = 0.15 ) and aseptic loosening (RR = 1.39,95% CI(0. 21,9. 39) , P = 0.74) of revision hip arthroplasty were not statistically significant. The modular stem reduced the subsidence( RR = 0.28,95% CI ( 0.08,0.95 ), P = 0.04 ) and postoperative pain ( RR = 0.45,95% CI ( 0.22, 0.94 ) ,P = 0.03 )in revision hip arthropiasty. [ Conclusion] Compared with coated long stem,the use of modular stem could sig-nificantly reduce the subsidence and postoperative pain in revision hip arthroplasty.
