摘要 目的探讨人工髋关节股骨假体在Vancouver B型骨折患者中的临床效果及对肢体功能的影响。方法选择2013年6月~2018年1月入院治疗的Vancouver B型骨折患者120例,所有患者均行人工髋关节置换术治疗,根据股骨假体不同分为对照组(n=60例)和观察组(n=60例)。对照组采用骨水泥型长柄股骨假体治疗,观察组采用生物型长柄股骨假体治疗,两组治疗后对患者进行3个月随访,对患者效果进行评估。记录并统计两组手术时间、出血量、住院时间及骨折愈合时间;采用髋关节Harris评分量表对两组治疗前、治疗后1个月、2个月及3个月肢体功能进行评估;记录并统计两组术后3个月下肢静脉血栓、内固定断裂、假体松动、骨水泥渗漏发生率,比较两组临床疗效及对肢体功能的影响。结果观察组术后3个月手术时间、出血量、住院时间及骨折愈合时间与对照组比较,差异无统计学意义(P>0.05);观察组治疗后1个月、2个月、3个月髋关节Harris评分,均高于对照组(P<0.05);观察组术后3个月下肢静脉血栓、内固定断裂、假体松动、骨水泥渗漏发生率,均低于对照组(P<0.05)。结论将人工髋关节生物型长柄股骨假体用于Vancouver B型骨折患者中有助于提高临床疗效,改善患者肢体功能,能降低术后并发症发生率,值得推广应用。 Objective To investigate the clinical effect of artificial hip femoral prosthesis in patients with Vancouver B type fracture and its influence on limb function.Methods One hundred and twenty patients with Vancouver B type fractures who were admitted to hospital from June 2016 to January 2018 were selected.All patients were treated with artificial hip arthroplasty.According to the different femoral components,they were divided into control group ( n =60cases) and observed.Group ( n =60cases).The control group was treated with cemented long stem femoral prosthesis.The observation group was treated with biological long-stem femoral prosthesis.After 2 months of treatment,the patient was followed up for 3 months and the effect of the patient was evaluated.The operative time,blood loss,length of hospital stay,and fracture healing time were recorded and calculated.The hip function was used to evaluate the limb function before and after treatment.The limb function was assessed.The incidences of venous thrombosis,internal fixation fracture,prosthesis loosening,and cement leakage in the two groups at 3 months after operation were compared.The clinical efficacy and effects on the limb function were compared between the two groups.Results There was no significant difference in operation time,blood loss,length of hospital stay and fracture healing time between the observation group and the control group at 3 months after operation ( P >0.05).The hip Harris scores at 1 month,2 months and 3 months after treatment in the observation group were all higher than those in the control group ( P <0.05).The incidence of thrombosis of the lower extremities,rupture of internal fixation,loosening of the prosthesis,and cement leakage in the observation group at the 3 months after operation were all lower than those in the control group ( P <0.05).Conclusion The use of artificial femoral hip prosthesis in Vancouver B type fractures can improve the clinical efficacy,improve the patient's limb function,and reduce the incidence of postoperative complications.It is worthy of popularization and application.
