摘要
目的探讨淋巴细胞输注治疗原因不明复发性流产的治疗效果及机理,为进一步推广应用提供理论依据和实验室佐证。方法应用淋巴细胞输注方法治疗原因不明复发性流产患者;应用ELISA法对治疗后患者血清HLA抗体进行检测;采用统计学方法分析抗体产生与否及产生抗体类型与治疗效果的相关性。结果 226名接受治疗患者中64.2%(145/226)产生HLA抗体,获得回访结果的159名患者中79.25%(126/159)治疗成功,HLA抗体转为阳性组的患者治疗成功率77.23%(78/101),抗体阴性组的患者治疗成功率82.76%(48/58),两者比较P>0.05,差异无统计学意义。只产生Ⅰ类抗体、只产生Ⅱ类抗体、Ⅰ+Ⅱ类抗体均阳性和抗体阴性4组病例比较治疗成功率总体没有统计学意义。结论淋巴细胞输注是治疗不明原因复发性流产的有效方法,能够刺激机体产生HLA抗体,检测HLA抗体尚不能作为评价和指导该治疗方法的有效指标。
Objective To discuss preliminarily the treatment mechanism and effect of lymphocyte infusion for patients with unexplained recurrent spontaneous abortion（ URSA）,and to provide theoretical basis and laboratory evidence for further promotion and application of this method,and also to provide theoretical support for more URSA patients who receive this method. Methods Lymphocyte infusion was applied to treat the URSA patients. ELISA was used in the detection of HLA antibodies of patients who received the treatment. Statistical analysis was used to analyze the level of antibodies,the type of antibodies,and the correlation with treatment effect. Results HLA antibody positive rate was 64. 2%（ 145 /226） in 226 patients who received the treatment. The success rate in the 159 return-visited patients was 79. 25%（ 126 /159）. The successful rate in HLA antibody positive patients and negative patient was 77. 23%（ 78 /101） and 82. 76%（ 48 /58）,respectively,with no significant difference between them（ P 0. 05）. There was no significant difference among the four groups,that is class Ⅰ antibody positive group,class Ⅱantibody positive group,both class Ⅰ and class Ⅱpositive group and antibody negative group. Conclusion Lymphocyte infusion was effective for URSA patients,which could stimulate the production of HLA antibodies. The positive rate of HLA antibody detected by ELISA could not be an effective indicator for evaluation of this treatment.
