右侧星状神经节阻滞对不停跳体外循环心内直视术患者脑氧代谢的影响 被引量:2

摘要 目的:探讨右侧星状神经节阻滞(sGB)对不停跳体外循环心内直视术患者脑氧代谢的影响。方法:择期行不停跳体外循环下心内直视术的患者60例,男38例,女22例,年龄20~65岁,体重42~67kg,ASAlI或Ⅲ级。采用随机数字表法,分为SGB组(S组)和对照组(C组),每组30例。S组行右侧SGB后麻醉诱导,C组直接麻醉诱导。于右侧颈内静脉逆行置管后即刻(基础状态,T0)、降温至鼻咽温32℃(T1)、复温至鼻咽温37℃(T2)、停体外循环时(T3)采集桡动脉和颈内静脉球部血,行血气分析,测定颈内静脉球血氧饱和度(SivO2)、动静脉血乳酸浓度,并计算桡动脉一颈内静脉球部血氧含量差值(Da—jvOz)、脑氧摄取率(CEO2)及桡动脉一颈内静脉乳酸差值(Da—jvL)。结果:两组各时点心率(HR),氧分压(SpO2),平均动脉压(MAP)和中心静脉压(CVP)比较差异无统计学意义(P〉0.05),且均在目标范围内。与C组比较,S组在T1~T3时SjvOz升高,Da—jvO2、CEO2和Da—jvL降低(均P〈0.05)。结论:右侧SGB可降低不停跳体外循环心内直视术患者的脑氧耗,保持脑血流和脑氧代谢的良好匹配,其机制需进一步探讨。
