摘要
目的:探讨甘精胰岛素联合不同口服降糖药治疗老年糖尿病的临床疗效及安全性。方法:将笔者所在医院收治的93例2型糖尿病老年患者按照分层随机分组法分为观察组与对照组,对照组给予甘精胰岛素+罗格列酮治疗,观察组给予甘精胰岛素+那格列奈治疗,观察两组糖化血红蛋白(HbA1c)、空腹血糖(FBG)、餐后2 h血糖(2 h PG)控制情况,同时观察低血糖发生率、血糖达标天数及胰岛素用量。结果:观察组血糖达标天数为(5.1±1.3)d,低血糖发生率为12.5%(7/56),对照组血糖达标天数为(8.3±1.5)d,低血糖发生率为32.4%(12/37),两组比较差异有统计学意义(P<0.05);观察组HbA1c及2 h PG控制情况也显著优于对照组,两组比较差异有统计学意义(P<0.05)。结论:胰岛素联合非磺酰脲类促胰岛素分泌剂那格列奈,能较好控制餐后血糖及降低低血糖发生率,具有重要临床应用价值。
Objective： To explore the clinical efficacy and safety of Insulin combined with the different oral hypoglycemic agents in the treatment of elderly patients with diabetes.Method： According to a stratified randomization method, 93 cases of elderly patients with type 2 diabetes were divided into the observation group and the control group in our hospital, the control group were given insulin glargine and rosigfitazone, the observation group were given insulin glargine and nateglinide, two groups of HbAlc, FBG, 2 h PG, the incidence of hypoglycemia, recovering time of blood glucose and insulin dosage were observed. Result： The blood glucose up to normal levels of time was （5.1 -＋ 1.3）d in the observation group and （8.3 ＋ 1.5）d in the control group（P〈0.05）, the incidence of hypoglycemia was 12.5% （7/56） in the observation group and 32.4% （12/37） in the control group（P〈0.05）, the observation group was significantly better than the control group in HbAlc and 2 h PG, the difference was statistically significant（P〈0.05）.Conclusion： Insulin combined nateglinide can significantly reduce the incidence of hypoglycemia and control the postprandial glucose levels, it has important clinical value.
