摘要
为了探讨三氧化二砷 (As2 O3 )对骨髓增生异常综合征 (MDS)粒单系细胞的体外作用 ,用半固体、液体培养及APAAP法检测不同浓度As2 O3 对 2 4例MDS患者CFU GM和CD33,CD15 ,bcl 2表达的影响。结果发现 ,As2 O3 随着浓度的增加对CFU GM的抑制作用增强 ;0 .388μmol/L时对丛的抑制大于集落 ,其中低危组 70 .78%对 34.0 5 % ,高危组 86.76%对 65 .86% (P <0 .0 1) ;0 .194 μmol/L时使低危组丛减少 ,而使集落增加 ,高危组丛减少 ,集落不变。高浓度 (1.94 μmol/L)时CD33与CD15表达降低 ,bcl 2表达减少 ,细胞出现核染色质固缩 ,胞浆浓缩 ;低浓度 (0 .194μmol/L)时低危组CD33减少 ,CD15增加 ,高危组CD33减少 ,CD15不变 ,bcl 2减少 ,且出现上述细胞形态学改变。由上述结果推测 ,高浓度As2 O3 抑制MDS粒单系造血 ,促使粒单系细胞凋亡 ;低浓度诱导低危组粒单系细胞增殖分化 ,抑制高危组粒单系细胞生长 。
The Effect of arsenic trioxide(As 2O 3) on myelomonocytic progenitor cells of patients with myelodysplastic syndrome was studied. Bone marrow CFU-GM was assayed in the agar semi-solid culture, and the bone marrow cells were incubated in liquid culture and the expressions of CD33, CD15 and bcl-2 on the marrow cells were detected by APAAP method in 24 patients. The suppressive effects of As 2O 3 to CFU-GM were increased with the rise of As 2O 3 concentrations. The suppression of As 2O 3(0.388 μmol/L) to cluster formation was stronger than to colony formation, the suppressive rates were 70.78% vs 34.05% in low-risk group, and 86.76% vs 65.86% in high-risk group(P<0.01), respectively. 0.194 μmol/L of As 2O 3 decreased clusters and increased colonies in low-risk group, but decreased clusters and did not change colonies in high-risk group. High concentration (1.94 μmol/L) of As 2O 3 downregulated the expression rate of CD33 and CD15 in both groups, and low concentration (0.194 μmol/L) downregulated the expression rate of CD33 and upregulated the expression rate of CD15 in low-risk group, but decreased expression of CD33 and did not alter CD15 in high-risk group. At the same time, the high concentration of As 2O 3 downregulated expression of bcl-2 and resulted in karyopyknosis and cytoplasm condensation; low concentration generated similar effect on expression of bcl-2 and cell morphology in high-risk group, but did not affect in low-risk. It is concluded that As 2O 3 suppressed myelopoiesis and impelled myelomonocytic cells to apoptosis, low concentration of As 2O 3 induced the proliferation and differentiation of myelomonocytic cells in low-risk group, however, suppressed the growth of myelomonocytic cells and accelerated the cells apoptosis in high-risk group.
