摘要
目的 观察用碘化油行肝动脉栓塞对集束电极射频治疗 (RFA)肝癌疗效的影响 .方法 1将猪随机分为两组 ,A组用碘化油行肝动脉栓塞后行集束电极热毁治疗 ,B组未行肝动脉栓塞直接行集束电极热毁治疗 ;2临床患者 ,将行肝动脉栓塞治疗者 (C组 )及未行肝动脉栓塞治疗者 (D组 )行集束电极射频热毁损治疗后分别统计 .结果 1离体猪肝的治疗时间为 (6 .0± 1.4) min,坏死直径 (5 .5± 0 .7) cm;2 A组的治疗时间为 (8.0± 1.3) m in,坏死直径 (4 .7± 0 .9) cm,B组的治疗时间为 (12 .0± 1.8) m in,坏死直径 (4 .0± 0 .6 ) cm(P<0 .0 1) ;3 C组的治疗时间 (9.0± 2 .1) min,坏死直径(5 .3± 1.4) cm,D组的治疗时间为 (16 .0± 4.6 ) min,坏死直径 (3.5± 1.8) cm(P<0 .0 1) .结论 肝动脉栓塞对集束电极射频治疗肝癌具有安全和增效作用 .
AIM To observe the effect of iodized oil on radiofrequency thermal ablation (RFA) of hepatic neoplasms by using a cluster array of ten separate electrodes. METHODS ①Twenty four small domestic pigs were divided randomly into 2 groups. In group A, radiofrequency thermal ablation of normal liver was given after transcatheter hepatic artery embolization (TEHA) with iodized oil; in group B, RFA was given without using TEHA; ②Clinic patients were divided into 2 groups: group C underwent transcatheter hepatic artery embolization; group D didn't undergo transcatheter hepatic embolization, and all radiofrequency ablation of hepatic neoplasms. RESULTS ① Time of RFA to ex vivo liver of pigs was (6.0±1.4) min, the diameter of coagulation necrosis optimal RFA was (5.5±0.7) cm; ② Time of RFA to group A was (8.0±1.3) min, the diameter of coagulation necrosis was (4.7±0.9) cm; the time of RFA to group B was (12.0±1.8) min, the diameter of coagulation necrosis was (4.0±0.6) cm ( P <0.01); ③ Time of RFA to group C was (9.0±2.1) min, the diameter of necrosis was (5.3± 1.4 ) cm, the time of RFA to group D was (16.0±4.6) min, the diameter of necrosis was (3.5±1.8) cm ( P <0.01). CONCLUSION Radiofrequency thermal ablation of hepatic neoplasms with transcatheter hepatic artery embolization using iodized oil might have synergiest effect.
