空心加压螺钉治疗青少年胫骨结节OgdenⅢ型撕脱性骨折 被引量:2

摘要 目的:探讨应用空心加压螺钉治疗青少年胫骨结节OgdenⅢ型撕脱性骨折的临床疗效.方法:2003年1月至2012年10月,采用切开复位内固定治疗青少年胫骨结节撕脱性骨折11例,其中男9例,女2例;年龄12.4~17岁,平均15.2岁;按照Ogden分型均为Ⅲ型,其中Ⅲa9例,Ⅲb2例.术后早期功能锻炼,同时定期随访,并按Mosier等评定标准对膝关节功能恢复进行评估.结果:11例均获随访,时间7~69个月,平均25.5个月.所有骨折获得Ⅰ期愈合.根据Mosier等评定标准对患儿膝关节功能进行评估,结果均为优.1例出现髌下感觉减退,1例出现胫骨结节明显凸出但无任何症状.结论:应用空心加压螺钉治疗青少年胫骨结节OgdenⅢ型撕脱性骨折,可获得满意疗效,患儿膝关节功能恢复正常. Objective:To review the clinical outcome of the application of cannulated compression screws for the treatment of tibial tubercle avulsion fractures of Ogden type Ⅲ in adolescents.Methods:From January 2003 to October 2010,11 adolescent patients with tibial tubercle avulsion fractures were treated with open reduction and internal fixation.There were 9 boys and 2 girls with an average age of 15.2 years old(ranged,12.4 to 17 years old).According to Ogden classification,there were 9 patients with type Ⅲa and 2 patien ts with type Ⅲb.Postoperative functional exercise and regular follow-up were done and outcomes were evaluated by the Mosier clinical assessment system.Results:All the patients were followed up,and the duration ranged from 7 to 69 months,with a mean of 25.5 months.All fractures healed at the first stage.According to the Mosier clinical assessment system,the final outcome was evaluated as excellent in all.One patient had infrapatellar hypoesthesia,and one patient had prominency of tibial tubercle without any symptoms.Conclusion:The cannualted compression screws in the treatment of tibial tubercle avulsion fractures of Ogden type Ⅲ in adolescents have an excellent outcome without deformity or functional loss.
