创面局部注射胰岛素对糖尿病足溃疡患者全身血糖及溃疡面微血管密度的影响 被引量:7

摘要 目的探讨创面局部注射胰岛素对糖尿病足溃疡患者全身血糖及溃疡面微血管密度的影响。方法对87例糖尿病足溃疡患者随机分为实验组以及对照组,对照组采用常规治疗,实验组患者将胰岛素1/2进行溃疡面注射,1/2进行腹部皮下注射。对两组患者的溃疡肉芽组织生长情况、CD34表达以及新生微血管密度等进行比较。结果两组患者在接受治疗前后其血糖变化无统计学差异(P>0.05);实验组患者在接受治疗后5、7、14 d,溃疡面肉芽生长率为(26.32±0.34)%、(61.21±1.43)%、(82.1±2.12)%,明显高于对照组的(18.43±0.89)%、(25.32±1.93)%、(41.2±3.29)%(P<0.05);实验组患者在接受治疗后5、7、14 d微血管密度分别为(9.21±0.34)、(11.39±1.12)、(14.93±1.19),明显高于对照组的(4.32±0.28)、(5.43±1.03)、(7.29±1.21)(P<0.05);实验组患者疗效优于对照组(P<0.05)。结论创面局部注射胰岛素对患者全身血糖有良好的控制效果,同时具有促进微血管生长,改善创面环境作用。
